The Intake Specialist is responsible for receiving referrals, entering and tracking intake information, completing initial outreach, following up on missing intake documents, and preparing clean handoffs for the next step in the approved PBHC intake workflow. This role is intake-focused and does not own provider matching, clinical assessment, case management assignment, billing decisions, or broad cross-department coordination.
The Intake Specialist serves as PBHC's front-end intake role for referral receipt, intake documentation tracking, initial client/referral-source outreach, service-type clarification, intake readiness updates, and structured handoff preparation under the operational access oversight of the CCO.
Intake support: the process of receiving, documenting, tracking, and following up on new client or referral information so the intake can move to the next approved step.
Referral follow-through: the communication and tracking needed to make sure referrals are not lost, delayed, duplicated, or left incomplete.
Client access support: the administrative intake work that helps clients move from referral to the appropriate access pathway without creating clinical, scheduling, or provider-assignment decisions outside the role.
