Position Overview:

The Case Manager will provide support and coordination of care to clients in a private counseling practice. The primary role of the Case Manager is to assess client needs, develop care plans, and ensure that clients receive appropriate services and resources to address their mental health, social, and emotional needs. The Case Manager will collaborate with therapists, medical providers, and community organizations to provide a holistic approach to client care, ensuring that clients can access the services necessary for their overall well-being.

 

Key Responsibilities:

Client Assessment & Care Coordination:

  • Conduct comprehensive client assessments to identify their social, emotional, and mental health needs.
  • Develop individualized care plans that address client needs, including coordination of services within the practice and referrals to external resources as needed.
  • Assist clients in navigating available services, including counseling, medical care, housing, financial support, or other social services.
  • Collaborate with therapists and other healthcare providers to ensure a cohesive approach to client care, including regular follow-up on treatment progress and adjustments to care plans as necessary.

Resource Management & Referrals:

  • Provide clients with information and referrals to community resources, social services, and support programs.
  • Facilitate communication between clients and external service providers, ensuring that clients receive the support they need in a timely manner.
  • Assist clients in securing financial assistance, housing resources, transportation, or other essential services that contribute to their recovery and stability.
  • Maintain relationships with external organizations and service providers to expand the network of resources available to clients.

Client Advocacy & Support:

  • Serve as an advocate for clients, helping them understand their rights and options for care, and ensuring they receive the services necessary to achieve their goals.
  • Provide support to clients and families in understanding and navigating healthcare systems, social services, and counseling options.
  • Assist clients with crisis intervention and immediate support as needed, connecting them with the appropriate resources in times of need.
  • Help clients develop self-management skills and coping strategies to support their mental health and overall well-being.

Documentation & Compliance:

  • Maintain accurate, up-to-date documentation of client interactions, care plans, referrals, and outcomes in accordance with practice standards and HIPAA regulations.
  • Ensure that all records are maintained confidentially and that care plans are regularly updated based on client progress.
  • Prepare reports and documentation for clinical meetings and case reviews as required.

Collaboration & Teamwork:

  • Work closely with therapists, counselors, and administrative staff to ensure that client care is coordinated and efficient.
  • Participate in clinical meetings, case reviews, and staff consultations to discuss client progress and identify potential improvements to care plans.
  • Engage in ongoing communication with the clinical team to ensure continuity of care for all clients.

Professional Development & Compliance:

  • Stay informed of best practices in case management and community resource availability.
  • Participate in continuing education opportunities and professional development to maintain knowledge of social services, mental health resources, and healthcare regulations.
  • Ensure compliance with all legal, ethical, and practice-specific requirements for client care and case management.

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master’s degree preferred).
  • Minimum of 2-3 years of experience in case management, social work, or related fields, preferably in a healthcare or counseling setting.
  • Strong knowledge of community resources, social services, and mental health care systems.
  • Excellent communication, organizational, and interpersonal skills, with the ability to work effectively with clients and healthcare professionals.
  • Experience working with diverse populations, including individuals facing mental health challenges, trauma, and social service needs.
  • Familiarity with electronic health records (EHR) and case management documentation processes.
