Senior Case Manager

Youth RISE Foundation

Expected Commitment: 10–15 hours per week

Location: Remote / Flexible

Youth RISE Foundation is seeking a Senior Case Manager to support young people ages 15–25 through individualized resource navigation, service coordination, advocacy, and ongoing support.

This opportunity is designed for someone with some experience in case management, social services, human services, youth development, community outreach, or a related field who wants to strengthen their professional skills while contributing to a growing organization.

The position is currently unpaid and does not guarantee compensation or future employment.

The expected commitment is approximately 10–15 hours per week, with flexibility around the individual's schedule.




About the Position

The Senior Case Manager provides direct case-management support to Youth RISE participants and helps coordinate access to appropriate community resources and services.

The role combines direct participant support, resource navigation, documentation, advocacy, referrals, and coordination.

The Senior Case Manager will work under the direction of the Executive Director of Social Services and collaborate with other departments when participant needs require additional organizational support.

This is an excellent opportunity for someone who wants to develop stronger skills in case management, social services, youth advocacy, service coordination, and nonprofit program delivery.




Core Responsibilities

Case Management

  • Conduct participant intake and needs assessments.
  • Develop individualized support plans with participants.
  • Maintain appropriate participant case records.
  • Monitor participant progress and identified needs.
  • Conduct appropriate follow-ups.
  • Help participants identify goals and available resources.
  • Coordinate services based on individual participant needs.

Resource Navigation

  • Identify appropriate community resources.
  • Connect participants with available services and organizations.
  • Maintain an updated resource directory.
  • Assist participants with navigating complex service systems.
  • Identify barriers to accessing services.
  • Help participants develop practical next steps.

Referrals

  • Make appropriate referrals to community organizations and service providers.
  • Maintain referral documentation.
  • Follow up when appropriate to determine whether participants successfully connected with resources.
  • Develop relationships with organizations that can support Youth RISE participants.

Advocacy

  • Advocate for participant needs within the appropriate scope of the position.
  • Help participants understand available resources and options.
  • Support participants in communicating their needs.
  • Identify systemic or recurring barriers affecting participants and communicate trends to Social Services leadership.

Documentation & Confidentiality

  • Maintain accurate and timely case documentation.
  • Protect participant confidentiality.
  • Follow Youth RISE privacy, documentation, and information-management policies.
  • Properly document referrals, contacts, assessments, and follow-ups.
  • Escalate significant concerns to the appropriate supervisor.

Collaboration

  • Work with the Executive Director of Social Services.
  • Collaborate with program staff and other departments.
  • Participate in case-management meetings when appropriate.
  • Communicate participant-related operational needs through appropriate channels.
  • Support multidisciplinary approaches when appropriate.



Senior-Level Responsibilities

In addition to direct case-management responsibilities, the Senior Case Manager may:


  • Support newer case managers with questions and best practices.
  • Assist with improving case-management workflows.
  • Help identify gaps in participant services.
  • Assist in developing case-management resources and procedures.
  • Contribute to departmental training materials.
  • Identify trends in participant needs.
  • Recommend improvements to Social Services leadership.
  • Assist with special projects and departmental initiatives.

The Senior Case Manager does not independently make clinical diagnoses or provide professional services requiring a specific license unless separately qualified, authorized, and legally permitted to do so.

Time Commitment

10–15 hours per week

This is intentionally structured around a manageable and flexible schedule.

The role does not require a traditional 9-to-5 schedule.

Scheduling can be coordinated around organizational needs and reasonable availability.

This position is currently unpaid and does not guarantee salary, wages, benefits, or future employment.

If Youth RISE Foundation develops the financial capacity to provide compensation for this or similar work in the future, any compensation would be subject to organizational approval, applicable law, and an appropriate written agreement.

Approved expense reimbursement or other permitted support may be available in accordance with Foundation policies.

Important Professional Boundaries

Youth RISE Foundation expects all case-management personnel to maintain appropriate professional boundaries and protect participant confidentiality.

Case managers are expected to operate within their training, experience, organizational policies, and applicable legal requirements.

This position is not a substitute for licensed clinical, medical, legal, or emergency services.

When a participant requires specialized or emergency assistance, the Senior Case Manager must follow established escalation and referral procedures.
