The Geriatric Social Worker plays a critical role in supporting clients and families facing aging- related transitions, chronic illness, dementia, caregiver stress, and psychosocial barriers to safe aging at home. This role blends clinical assessment, care coordination, caregiver support, and community resource navigation, working closely with families, care managers, and our caregiving teams. This position is ideal for a social worker who values autonomy, flexibility, interdisciplinary collaboration, and mission-driven work — without the bureaucracy of traditional institutional settings.

Key Responsibilities

Clinical & Family Support

  • Conduct psychosocial assessments for older adults and family caregivers
  • Support clients and families through transitions such as functional decline, dementia

progression, hospitalization, or increased care needs

  • Provide caregiver counseling, education, and emotional support
  • Assist with advance care planning conversations and resource education

Care Coordination & Navigation

  • Collaborate with Woven care leaders, caregivers, and external providers
  • Identify and connect families to appropriate community resources (housing, benefits,

counseling, support groups, etc.)

  • Support discharge planning and changes in level of care
  • Help families problem-solve barriers to safe aging in place

Documentation & Quality

  • Maintain timely, accurate documentation in electronic systems
  • Participate in interdisciplinary case reviews and care planning
  • Contribute to continuous quality improvement initiatives

Community & Mission Engagement

  • Serve as a trusted resource to families, referral partners, and community organizations
  • Support education efforts related to aging, caregiving, and dementia when needed

Qualifications

Required

  • Master’s Degree in Social Work (MSW)
  • Licensed or license-eligible in Missouri (LMSW or LCSW)
  • Experience working with older adults, caregivers, or healthcare-adjacent populations
  • Strong interpersonal, communication, and organizational skills
  • Valid driver’s license and ability to travel locally

Preferred

  • Experience in geriatrics, dementia care, hospice, home care, or care management
  • Familiarity with social determinants of health and community-based resources
  • Comfort working independently while collaborating with a team
  • Interest in innovative, home-based care models
