This position is a remote Nurse Practitioner role providing wound, incision, and ostomy care to patients through virtual/telehealth services. The Nurse Practitioner will evaluate patients remotely, develop and manage individualized treatment plans, monitor wound progress, and provide ongoing clinical guidance and education to patients, caregivers, and other members of the healthcare team. The Nurse Practitioner will collaborate closely with patients, caregivers, home health providers, facility staff, and other healthcare professionals to support continuity of care, timely interventions, and optimal clinical outcomes. The role may support patients across multiple care settings, including homes, assisted living facilities, skilled nursing facilities, outpatient settings, and other community-based programs. The position requires strong clinical assessment, communication, organization, and documentation skills, as well as the ability to effectively manage patient care through a virtual platform.
Education: Master’s Degree
Master’s degree in Nursing from an accredited nursing program. Licensed as a Registered Nurse by the State Board of Nursing (SBON) in the state(s) where the nurse practices. Board certified and licensed as a Nurse Practitioner in the state(s) where the Nurse Practitioner practices.
Specialty Education/Certification:
Preferred: Completion of a WOCN Society-accredited educational program and current certification by the WOCNCB in wound care, including one of the following:
