Education: Master’s Degree

Master’s degree in Nursing from an accredited nursing program. Licensed as a Registered Nurse by the State Board of Nursing (SBON) in the state(s) where the nurse practices. Board certified and licensed as a Nurse Practitioner in the state(s) where the Nurse Practitioner practices.

Specialty Education/Certification:

Preferred: Completion of a WOCN Society-accredited educational program and current certification by the WOCNCB in wound care, including one of the following:

• CWCN – Certified Wound Care Nurse
• CWON – Certified Wound Ostomy Nurse
• CWOCN – Certified Wound Ostomy Continence Nurse

Alternative:

• WCC – Wound Care Certified through the National Alliance of Wound Care and Ostomy 
• CWS – Certified Wound Specialist through the American Board of Wound Management

Minimum Experience:

Nurse Practitioner: Minimum of one (1) year of experience in a wound-related Nurse Practitioner role with one of the following

• Three (3) years of experience as a CWCN, CWON, or CWOCN
• Five (5) years of experience as a WCC or CWS

Roles and Responsibilities:

• Provide patient consultation, evaluation, and management via telehealth, virtual care, or digital monitoring modalities for patients with incisions, wounds, ostomies, and related skin disorders.
• Provide virtual clinical support across The Wound Company’s programs and geographic markets based on patient and program needs.
• Patient volume and workload expectations will vary based on program needs and may include scheduled telehealth visits, virtual clinical review, patient follow-up, care coordination, documentation, and clinical support activities.
• Support enrolled patients through assessment, treatment planning, follow-up, patient education, care coordination, and monitoring of clinical progress.
• Review available clinical information, including wound photographs, patient history, assessment findings, diagnostic information, and information from other members of the healthcare team, to support appropriate clinical decision-making.
• Documentation of services to support the appropriate level of billing consistent with CMS guidelines. Documents clearly and concisely via the EMR and completes documentation in real time whenever reasonably possible.
• Provider completes all billable and non-billable service documentation within one (1) business day.
• Complete required orders, care plans, follow-up documentation, and other clinical documentation within established company timeframes.
• Use judgment and critical-thinking skills to assess, diagnose, and identify appropriate outcomes and interventions to promote safe patient engagement and positive clinical outcomes.
• Use evidence-based assessment techniques, tools, clinical guidelines, and available data and information to identify problems and needs of patients with wounds, ostomies, incisions, and related skin disorders.
• Collect pertinent biological, physical, functional, and psychosocial data using a systematic process to identify patient needs.
• Prescribe wound, ostomy, and incision care within scope of practice, including treatments, medications, supplies, and other appropriate interventions.
• Collaborate with Clinical Support Coordinators, clinical specialists, field-based providers, and other members of the clinical team to coordinate care and delegate appropriate pre-visit and follow-up activities based on patient needs.
• Provide clinical direction and guidance to Clinical Support Coordinators and other members of the care team as appropriate to support timely and coordinated patient care.
• Facilitate and coordinate care with other providers, including home health, primary care, specialists, skilled nursing facilities, assisted living facilities, caregivers, and other members of the patient’s healthcare team, through the EMR and approved digital communication modalities to enhance care and prevent and manage complications for enrolled patients.
• Communicate respectfully, clearly, and concisely with patients, caregivers, providers, facilities, Clinical Support Coordinators, clinical specialists, and other team members.
• Support field-based Nurse Practitioners and other clinical team members with patient-specific clinical questions, care coordination, follow-up needs, and other clinical support as requested.
• Obtain additional state licensure when requested by The Wound Company and when reasonably necessary to support company programs. The Wound Company will cover approved licensing expenses.
• Evaluate processes, policies, procedures, and protocols/guidelines for the care of patients with incisions, wounds, ostomies, and related skin disorders with other team members as requested.
• Recommend revisions to processes, policies, procedures, and clinical protocols/guidelines when warranted by evidence, regulatory requirements, clinical practice, or operational needs.
• Participate in program development of the Nurse Practitioner program, including review and improvement of processes to support billing, documentation, patient outcomes, and quality measures.
• Provide leadership and clinical input in the design and implementation of quality-improvement initiatives to optimize outcomes of care for patients.
• Participate in collaborative meetings with team members to support existing and new programs.
• Participate in clinical program development and implementation, including development and refinement of workflows, processes, and clinical standards.
• Communicate anticipated schedule changes, time away, and coverage needs in accordance with company workflows to support continuity of patient care.
• Maintain current wound-care certification and Nurse Practitioner licensure/certification and participate in ongoing educational opportunities to remain current and practice as an expert in the field.
• Maintain current Registered Nurse licensure and Nurse Practitioner national board certification as applicable to the provider’s practice.
• Uphold company policy for HIPAA compliance to maintain patient privacy, confidentiality, and data security.
• Has and maintains access to high-speed internet at home at the level necessary to complete virtual clinical responsibilities.
• Maintains document destruction measures for the home office.
• Maintains a private and appropriate workspace for virtual clinical responsibilities and protection of patient information.
• Maintains company equipment and supplies in a clean, secure, and working condition.
• Completes other duties as assigned.
