Education: Master’s Degree
Licensed as a Registered Nurse by the State Board of Nursing (SBON) in the state(s) where the nurse practices. Board Certified as Nurse Practitioner in the State(s) where the Nurse Practitioner practices.
Specialty Education/Certification:
Preferred: Completion of a WOCN Society accredited educational program and certification by the WOCNCB in wound care. CWCN (Certified Wound Care Nurse)
Alternative: Wound Care Certified, Certified Wound Specialist with a minimum of 3 years working as a wound specialist
Minimum 3 years full time experience in Acute Care, Skilled Nursing Facility, Home Health, or Outpatient clinic practicing as a certified wound specialist and a minimum of 1 year practicing as a Nurse Practitioner in a wound related role.
Roles and Responsibilities:
- Provide in-person evaluation and management visits for patients with acute and chronic wounds and related skin disorders within a defined geographic area in home setting, assisted living and/or skilled nursing facilities. Perform medically necessary debridement and procedures within CMS guidelines to promote healing. Estimated time spent on in-person care 80%
- Provide evaluation and management visits via telehealth or digital monitoring modalities for patients with acute and chronic wounds and related skin disorders. Estimated time for virtual care 20%
- Follows appropriate level of infection prevention protocols and handling of contaminated equipment.
- Documentation of services to support appropriate level of billing consistent with CMS guidelines. Documents in real time clearly and concisely via EMR specific to enrolled patients. Provider completes billable services documentation within 1 business day.
- Uses judgement and critical thinking skills to assess, diagnose and identify outcomes to promote safe patient engagement.
- Uses evidence-based assessment techniques, tools and available data and information to identify problems and needs of a patient with acute and chronic wounds and other related skin disorders.
- Collects pertinent data (biological, physical, functional and psychosocial) using a systematic process to identify the patient’s needs.
- Prescribes care within scope of practice including treatments and medications.
- Collaborates with virtual and field-based clinical specialists to coordinate care and delegates pre-visit and follow up care based on patient needs.
- Facilitates and Coordinates care with other providers (i.e. home health, primary care, etc.) via EMR and digital modalities to enhance care and prevent and manage complications for enrolled patients.
- Communicates clearly and concisely with other team members.
- Evaluate processes, policies, procedures and protocols/guidelines for care of patients with acute and chronic wounds and other related skin disorders with other team members as requested. Recommends revisions when warranted.
- Participates in program development of nurse practitioner program including review of processes to support billing, documentation and quality measures.
- Provides leadership in the design of quality improvement initiatives to optimize outcomes of care for patients.
- Participates in collaborative meetings with team members to support existing and new programs.
- Maintains certification as CWCN or other related wound certification and licensure as nurse practitioner and participates in ongoing educational opportunities to stay current and practice as an expert in the field.

- Has and maintains access to high-speed internet at home at the level necessary to complete virtual clinical responsibilities.
- Has and maintains cellular phone plan.
- Maintains document destruction measures for home office.
- Has reliable means of transportation and maintains automobile insurance for onsite and in person assessments.
