Working Partnerships USA is seeking to hire a Healthcare Navigation Team to help support scheduling intake appointments with local community Health Clinics. This team will be trained to have conversations with patients under HIPAA privacy regulations and the proper document security measures taken with any confidential information. Patients everywhere are being affected and are confused by the major changes happening to Medi-Cal, making some patients lose their healthcare coverage in the process. This work will directly support vulnerable populations who are being affected by these new rules and changes, directly helping families to meet deadlines and receive benefit application assistance. This team will also support community engagement efforts, engaging and educating community members , encouraging them to join efforts to share their healthcare experiences, attend health resource fairs with their neighbors, learn about care for the community, and participate in collective action.

Key Responsibilities

  • Navigators will primarily work from the office, calling patients and scheduling intake appointments.
  • Navigators will participate in daily briefings/debriefings, trainings, and other skills-building activities 
  • Navigators will be expected to reach daily and weekly benchmark goals for the number of contacts made and intakes scheduled. 
  • Navigators will maintain organization and accuracy when recording phoning activities or maintaining confidential materials


Qualifications

  • Must be available from September 1st 2026 - January 31st 2027. 
  • Must be bi-lingual in a language representative of Santa Clara County, including but not limited to: Spanish, Vietnamese, or Mandarin. Able to translate and communicate to patients in a different language. 
  • Must have strong written and verbal communication skills as well as ability to connect with diverse people, always maintaining a positive and enthusiastic attitude.
  • Must be able to use a cellphone, laptop, zoom/video conferencing, a web browser.
  • Must have a strong commitment to social and racial justice.

PREFERRED

Compensation & Location


Our office is located in North San Jose. This is a temporary, part-time, non-exempt position. Salary Range between $28 - $30/hour 20-25 hours per week during Monday -Friday.

 

The position will be open until filled and starts immediately.
