About the Role
The Lead Care Manager (LCM) is an integral part of Wildflower’s multidisciplinary Enhanced Care Management (ECM) team and is the primary point of contact for pregnant and postpartum members enrolled in ECM under the Birth Equity Population of Focus (POF). The LCM is responsible for conducting outreach and comprehensive assessments, co-developing individualized care plans, and engaging members in their care. They provide evidence-based guidance, support, and care navigation services along with coordinating care with the member’s care team, health plan, and community-based perinatal services.
The ECM Lead Care Manager will report to the Health Advocacy Manager and, as needed, to the ECM Director. This individual will work closely with the ECM Clinical Consultant and other members of the ECM team at Wildflower. Salary Range: $60,000 to $70,000 based on experience
Schedule: Monday to Friday 8 AM to 5 PM Pacific Time
Work Location: Hybrid, must reside in either LA County or Kern County, California
