About the Role

The Health Coach is responsible for providing women's health and maternity coaching across multiple domains, including family building, pregnancy, and postpartum. Key responsibilities include: supporting patients in cultivating internal motivation and confidence, providing resources and strategies to start and sustain behaviors that align with provider care plans and patient goals for optimal health, collaborating with Health Advocates to ensure that patients have the resources they need. The Health Coach will work with individuals across a variety of geographies and social demographics to improve patient health and support optimal patient outcomes.

 

The Health Coach will coordinate with the head of Health Management services. This individual will work closely with other coaches and members of the Health Management team.  This is a fantastic opportunity for a caring individual who enjoys working with a growing company, is passionate about improving health outcomes, and is committed to creating exceptional experiences for patients.

 

Key Responsibilities

  • Coach patients across social and clinical needs with the ability to create strong interpersonal relationships
  • Leverage patient data to inform goal setting for lifestyle and health improvement
  • Leverage evidence-based motivational interviewing and behavior change techniques to promote healthy lifestyle choices, improve care plan adherence, and overall health and wellness through motivation, guidance, support, and encouragement
  • Work collaboratively with the Health Management team, including Health Advocates, to support care coordination and help patients achieve their goals 
  • Help patients create and implement effective solutions to improve their health and wellness 
  • Leverage general knowledge base of women's health, maternal health, pediatric health, emotional health, nutrition, and/or fitness to support patients and their families
  • Help patients identify areas of behavior and habit changes to help support optimal health and wellness to improve outcomes
  • Empower patients to take an active role in their health and healthcare needs
  • Facilitate coaching sessions with patients via phone, video, and email 

 

Qualifications:

  • Bachelor’s degree required - preferably in Psychology, Social Work, Public Health or another health-related field or equivalent combination of education and experience
  • Current NBC-HWC, NCHEC, or similar coaching certification
  • 2-3 years of health and wellness coaching experience required
  • Motivational Interviewing and knowledge on behavior change techniques required
  • Bilingual - written and verbal communication skills in English and Spanish required 
  • Understanding of women's health including nutrition, fertility, pregnancy, and postpartum strongly preferred
  • Experience in trauma informed care, health equity, and/or social justice preferred 
  • Experience working with diverse populations seeking healthcare and cultural competency preferred 
  • Knowledge of some medical terminology preferred
  • Experience coaching remotely by phone and/or video conferencing preferred
  • Strong judgment regarding scope of work and in determining when to refer to appropriate resources and qualified providers
  • Proficiency using software programs such as Microsoft Word, Excel, and Outlook or similar software

  • Maintains and ensures the security/confidentiality of all personal health information (PHI) collected
  • Excellent verbal, written and interpersonal communication skills
  • Strong organizational and time-management skills
  • Maintain accurate and timely records of coaching sessions, progress, and personalized plans
  • Familiarity with startup environments
  • Demonstrate a strong commitment to the mission and values of the organization

Job Type: Part-time Contractor

Pay: $50 per coaching session (usually 20-30 minute sessions)

Expected hours: 10 – 20 per week

Schedule: Monday - Friday availability required, weekend availability preferred - contractor schedule will depend on volume of appointment requests

Work Location: Remote, West Coast

Salary

$50 - $50 per hour
