Job Title: Integrated Behavioral Health Provider
Job Type: Full-Time in a Rural Primary Care Clinic
Location: Cumberland, Maryland
Reports To: Chief Medical Officer
Position Summary:
We are seeking a compassionate and skilled Mental Health Provider to join our integrated care team in a primary care clinic. The provider will deliver evidence-based behavioral health interventions to patients across the lifespan, supporting whole-person care in collaboration with primary care providers. This position plays a key role in addressing the mental health needs of an underserved community by improving access, reducing stigma, and supporting population health.
Key Responsibilities:
- Provide short-term, solution-focused behavioral health services within the primary care setting using a collaborative and integrated behavioral health model.
- Conduct assessments, diagnoses, and treatment planning for mental health and behavioral concerns (e.g., depression, anxiety, trauma, etc).
- According to level of training, deliver evidence-based therapies (e.g., CBT, MI, ACT) in 15-30 minute sessions tailored to the fast-paced primary care environment.
- Offer crisis intervention, safety planning, and referrals to specialty mental health or substance use treatment when appropriate.
- Work closely with primary care providers and care managers/community health workers to address social determinants of health and support behavioral change.
- Document encounters in the electronic health record (EHR) in a timely and accurate manner.
- Participate in team meetings, case reviews, and clinical quality improvement initiatives.
- Engage in community outreach or education to reduce stigma and increase mental health awareness.
Qualifications:
- Graduate of an accredited Nurse Practitioner or Psychiatry/Behavioral Health program
- Licensure & Certification:
- Current and unrestricted state license (e.g., CRNP-PMH, LCSW, LCPC, LMFT, PsyD, PhD)
- BLS certification preferred
- Eligible for credentialing with public and private insurance payers
- 1–3 years of experience in outpatient behavioral health, primary care integration, or community mental health
- Familiarity with rural or underserved populations is strongly preferred

- Ability to manage mental health conditions with medication intervention
- Ability to function effectively in a fast-paced, team-based care environment

- Proficiency in EHR systems and documentation workflows
- Flexibility, resourcefulness, and comfort working in a lower-resource rural setting
Work Environment:
- Rural outpatient clinic with integrated primary and behavioral health services
- May include telehealth service delivery as appropriate
