Job Title: Advanced Practice Provider (NP/PA)

Reports To: Chief Medical Officer
Position Summary:
We are seeking a dedicated, skilled, and compassionate Advanced Practice Provider to join our rural family medicine clinic. Our providers will deliver full-spectrum primary care services to patients of all ages, with a focus on health promotion, disease prevention, and diagnosis and management of acute and chronic illnesses in a patient-centered environment. The ideal candidate is self-motivated, adaptable, and committed to practicing in a collaborative, team-based setting.
Key Responsibilities:
- Provide comprehensive outpatient primary care to individuals and families across the lifespan.
- Conduct physical examinations, obtain medical histories, and order or interpret diagnostic tests.
- Diagnose and treat common acute and chronic medical conditions.
- Develop and manage treatment plans, prescribe medications, and make referrals to specialists when needed.
- Perform in-office procedures (e.g., suturing, skin lesion removal, joint injections, women’s health procedures) as appropriate to training and scope.
- Educate patients and families on wellness, prevention, and disease management.
- Collaborate with other clinical staff, including medical assistants, behavioral health providers, and specialists.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Participate in quality improvement initiatives, team meetings, and continuing medical education (CME) activities.
- Adhere to evidence-based clinical guidelines and standards of care.
Qualifications:
- Required Education:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Licensure & Certification:
- Current and unrestricted state license as an NP or PA AND National certification (e.g., AANP, ANCC, NCCPA)
- Eligible for or currently enrolled in collaborative/supervisory agreement as required by state law
- AND
- DEA license and BLS/ACLS certification
- Preferred Experience:
- 1–3 years of clinical experience in primary care preferred, but new graduates welcome depending on setting
- Experience working with diverse populations or in community health preferred

Work Environment:
- Outpatient clinical setting
- Monday through Friday standard working hours with one late night from 10-7
- No weekend, holiday, or on-call duties
