Direct Primary Care Nurse Practitioner (or PA)

The Role

We're looking for a seasoned NP or PA who is done with rushed visits and insurance-driven volume — and ready to actually practice medicine the way you were trained to.

You'll build and manage your own panel of direct-pay patients across primary care, weight management, and women's health — on a schedule you design and largely asynchronous communication with your patients. No churn-and-burn scheduling. Deliver care the way you actually want to practice, not the way a system dictates.

Start: 6–10 hrs/week, $65–$72/hr during onboarding Growth: Pay increases automatically as your panel grows transition to salary + a per-patient bonus once your panel exceeds 50 adult-equivalent patients

Support: Virtual assistant + in-office medical assistant, so your time goes to patients, not paperwork. AI dictation.

What You'll Do

• Manage a longitudinal panel across the full lifespan: assessment, diagnosis, and treatment of acute and chronic conditions
• Run comprehensive women's health care (wellness exams, contraception management, gynecologic screening) and evaluate men's/women's health concerns including perimenopause and reproductive health
• Build and monitor individualized medical weight management plans, including prior authorization support for GLP-1 and other medications where required
• Provide preventive care and health coaching tailored to each patient's goals and lifestyle
• Coordinate with specialists and escalate to our advisory/specialty programs for complex cases as needed
• Prescribe within VA NP/PA scope and DEA/controlled substance guidelines
• Document encounters in Optimantra (our EMR) within 24 hours; respond to patient messages within 24 hours (in-person, virtual, and secure messaging visits). Messages are triaged initially by MA.

What You Need

Required

• Active, unrestricted VA license + national board certification
• Active DEA license (or documented eligibility to obtain before start)
• 3+ years as a licensed APP or physician in primary care, family practice, internal medicine, urgent care, ER, functional medicine, or women's health
• Current BLS/CPR

Strong Plus

• Direct-pay, cash-pay, DPC, or concierge experience
• FNP autonomous practice authority (VA)
• GLP-1 / metabolic weight loss protocol experience
• Joint injections or other MSK procedural skills
• Bilingual (Spanish/English)
• Comfortable with telehealth and AI-assisted documentation
