Community Support Specialist

Reporting to the PS/CCSS Director, the Community Support Specialist will play a vital role in providing personalized care to clients, ensuring their needs are met in a timely and effective manner. Responsibilities include assessing, planning, implementing, monitoring, and evaluating actions to address the health and human service needs of assigned clients.

Key Responsibilities:

- Coordinate and deliver client-centered care

- Manage case assignments, develop service plans, monitor progress, and provide feedback for case closure

- Establish and maintain effective working relationships throughout the case management process

- Adhere to professional standards and regulations

- Evaluate clients, develop care plans, and assist with treatment plans

- Conduct thorough assessments and address motivational and psychosocial issues

- Facilitate investigations, referrals, and legal proceedings

- Collaborate with agencies and service providers for case coordination

- Provide education and support to help clients understand their conditions and treatment options

- Maintain accurate client records and ensure they are well-organized

- Connect clients with necessary resources and supports

- Accompany clients to appointments and activities as needed

- Assist clients in developing treatment plans and offer guidance on daily life skills

Qualifications:

- Previous experience in case management or related healthcare roles

- Knowledge of case management principles, healthcare management, and reimbursement

- Familiarity with psychological aspects of care

- Strong communication, organizational, and problem-solving skills

- Compassionate with the ability to work effectively in a team

- Current license, certification, or registration in a related healthcare field

- Bachelor's degree in a related field

Preferred Experience:

- Experience working with individuals with mental health or substance abuse issues

- Experience in public assistance programs

Physical Requirements:

- Ability to sit, stand, bend, and lift up to 15 pounds

- Must be able to move and lift objects as needed
