Reports To: VP of Operations/Administrator


Employment Type: Full-Time (Hybrid)


  • Remote: 3–4 days/week
  • Office: 1–2 days/week (Riverview/Tampa office)

• • Community travel required throughout Hillsborough, Pasco, Pinellas, Polk, Manatee, Sarasota, Hernando, Citrus, and surrounding service areas.

Position Summary


Unique Home Care is seeking a compassionate, outgoing, and highly organized Community Liaison & Client Coordinator to serve as the face of our agency. This position is responsible for building community relationships, coordinating new client admissions, providing exceptional customer service, and creating an outstanding client experience from the first phone call through ongoing services.


The ideal candidate enjoys networking, building relationships, solving problems, and making families feel supported during life’s most important moments.


Essential Responsibilities


Community Relations & Business Development


  • Develop relationships with hospitals, physicians, rehabilitation facilities, senior centers, assisted living communities, case managers, discharge planners, faith-based organizations, and community partners.
  • Represent Unique Home Care at networking events, health fairs, and community outreach activities.
  • Conduct routine visits to referral sources to strengthen partnerships.
  • Promote agency services throughout the community.
  • Maintain referral tracking and follow-up documentation.
  • Identify opportunities to increase private-pay referrals.


Client Intake & Coordination


  • Respond promptly to all client inquiries.
  • Explain agency services and answer questions.
  • Conduct initial intake interviews.
  • Schedule in-home assessments with the Administrator or designee.
  • Coordinate the onboarding process for new clients.
  • Maintain accurate client records within AxisCare.
  • Ensure all required intake documents are completed.



Client Experience


  • Conduct welcome calls for all new clients.
  • Complete routine satisfaction calls with clients and families.
  • Address non-clinical client concerns promptly and professionally.
  • Coordinate communication between clients, caregivers, supervisors, and leadership.
  • Assist families with service changes and scheduling requests.
  • Promote client retention through exceptional customer service.



Marketing & Community Engagement


  • Assist with social media engagement.
  • Encourage Google Reviews and client testimonials.
  • Assist with agency marketing campaigns.
  • Distribute promotional materials throughout the community.
  • Help coordinate community events and educational presentations.



Qualifications


Required


  • High School Diploma or GED
  • Minimum two years of healthcare, customer service, or community outreach experience
  • Excellent communication skills
  • Strong organizational abilities
  • Computer proficiency (Microsoft Office)
  • Reliable transportation
  • Valid driver’s license

  • CNA
  • Home Care experience
  • Patient Advocate experience
  • Case Management or Care Coordination experience
  • Marketing or Business Development experience
  • Bilingual (English/Spanish) preferred
