This is not traditional or general case management. Our ICMS Case Managers work alongside individuals who have experienced or are still experiencing homelessness and may be navigating complex, overlapping needs related to housing stability, physical and behavioral health, substance use, benefits, community integration, and other systems of care.
The work requires someone who understands the intensity of ICMS, can build genuine and trusting relationships with participants, and can navigate complex situations while remaining organized, responsive, collaborative, and grounded in person-centered, trauma-informed, and harm-reduction practices.

Role Overview
As an ICMS Case Manager, you will provide ongoing intensive case management to an assigned caseload of no less than 25 participants.
You will serve as a primary point of connection for participants, regularly assessing needs, coordinating services, supporting housing stability and holistic wellness, identifying gaps in care, and collaborating with internal teams and external providers.
This role includes both office and field-based work and requires the ability to work independently while remaining closely connected to the broader ICMS team and participant care network.
Schedule: Full-time, Monday-Friday, 36-hour workweek
Compensation: $33.61/hour
Employment Status: Temporary, full-time, non-exempt
Key Responsibilities
- Develop meaningful, trust-based relationships with participants using person-centered, trauma-informed, motivational interviewing, and harm-reduction approaches.
- Manage an assigned ICMS caseload of no less than 25 participants with complex and potentially high-acuity needs.
- Provide ongoing intensive case management, including regular participant contact, assessment of needs, service coordination, advocacy, and follow-up.
- Support participants in maintaining housing stability and connecting with physical health, behavioral health, substance use, benefits, community, and other appropriate services.
- Conduct participant visits and provide case management services in the field as required by participant needs and program expectations.
- Identify service gaps and coordinate with community providers and partners to support continuity of care.
- Collaborate with internal and external multidisciplinary partners involved in participant care.
- Maintain timely, accurate, and thorough case documentation, records, tracking, and required program reporting.
- Navigate participant crises, changing needs, and complex circumstances while maintaining appropriate professional boundaries and following organizational protocols.
- Attend required team meetings, trainings, case conferences, retreats, and professional development activities.
- Maintain consistent and effective communication with participants, team members, supervisors, service providers, and other collaborators.
Additional Responsibilities
- Conduct tours as needed or requested.
- Attend seminars, conferences, and professional development trainings as required.
- Participate in organizational and program activities as appropriate.
- Perform other duties related to the role as assigned.
Required Experience
- Previous professional experience providing Intensive Case Management Services (ICMS) is required.
- Candidates should have direct experience working within an intensive case management model serving individuals with complex needs. General case management experience alone, without relevant intensive case management experience, may not meet the requirements of this position.
- Experience working with individuals experiencing or transitioning out of homelessness is strongly preferred.
- An equivalent combination of relevant education, specialized training, and direct ICMS experience may be considered.
Skills & Abilities
- Strong understanding of intensive, person-centered case management.
- Ability to effectively manage a complex caseload while maintaining timely follow-up and documentation.
- Strong interpersonal and relationship-building skills, particularly with individuals who may have experienced significant trauma, instability, or systemic barriers.
- Knowledge and application of trauma-informed care and harm-reduction practices.
- Ability to navigate challenging or rapidly changing situations with sound judgment and appropriate professional boundaries.
- Strong organizational, planning, communication, and record-keeping skills.
- Ability to collaborate effectively across multidisciplinary teams and community service systems.
- Proficiency with standard computer applications, electronic documentation, spreadsheets, email, and internet-based research.
Licenses & Requirements
- Valid California driver's license or equivalent.
- Ability to travel locally as required to provide field-based participant services.
- Successful completion of required background screening.
Working Conditions
This position includes both office and community-based work. Team members may travel throughout the Los Angeles area to meet participants, coordinate services, and support participant needs.
The role may involve walking, climbing stairs, working indoors and outdoors in varying weather conditions, and occasionally lifting up to 20 pounds.
Temporary Employment & Benefits
- This is a temporary, full-time position and does not guarantee employment for a specific duration or future conversion to regular employment.
- Temporary employees are not eligible for The Center's employer-sponsored medical, dental, vision, retirement, or other benefits offered specifically to regular full-time employees.
Temporary employees currently receive:
- 13 paid holidays annually
- 24 hours of vacation every three months
- 36 hours of sick time, in accordance with organizational policy and applicable law
