Role Purpose
As a Caregiver Advocate at TCARE, you will be the face of our program, driving engagement and providing invaluable support to caregivers. Your role will involve managing outreach efforts, facilitating caregiver enrollment, and connecting them with the TCARE Specialist team. This is a unique opportunity to make a significant difference in the lives of caregivers while working in a collaborative and impactful environment.
Key Responsibilities

Be a TCARE Ambassador: Represent and promote TCARE's values and services with enthusiasm and integrity, serving as the first point of contact for caregivers.

Engage and Support: Conduct outbound and inbound communications to recruit, conduct screenings to identify specific caregiver needs, offering personalized support and education about the TCARE® Program and services.

Build Relationships: Establish and nurture strong, trusting relationships with caregivers, ensuring a positive and supportive experience.

Coordinate and Schedule: Manage appointments and coordinate with the Caregiver Specialist Team to ensure caregivers receive timely and effective support.

Educate and Inform: Provide caregivers with essential program information and resources through various communication channels, including print, email, and SMS.

Collaborate and Innovate: Work closely with the Caregiver Specialist Team and community organizations to identify and secure necessary resources and contribute to the continuous improvement of our processes.
Qualifications

Experience: Minimum of 2 years in community-based services, public health, or a related field with experience in conducting screenings or making referrals.

Skills: Exceptional customer service, communication (both written and verbal), organizational, and time management skills.

Empathy and Compassion: Ability to handle sensitive conversations with empathy and a genuine desire to support caregivers.

Technical Proficiency: Comfortable using standard software packages, including Teams and Microsoft Office (Outlook, PowerPoint, Excel).

Independence and Initiative: Capable of working independently, managing multiple tasks, and meeting job expectations and client needs.
Preferred Experience
- Experience working with social workers or in caregiving support roles.
- Background in supporting older adults, individuals with intellectual or developmental disabilities, Disabled American Veterans, or family caregivers.
- Bilingual (Spanish fluency) desired
