Key Responsibilities

• Conduct daily clinical rounds; review vitals, wounds, mobility, intake, and behavioral status
• Perform nursing assessments and document findings in SOAP form in Tebra
• Reconcile medications members bring with them; verify secure self-storage
• Respond to and triage escalations from Support Specialists; determine level of response
• Determine when a member requires hospital transfer and initiate it
• Participate in the paid after-hours on-call rotation
• Complete the care plan Baseline at the first available visit following admission
• Review and sign the care plan at least every 14 days, and on any change in condition, authorization, or destination
• Assess and document clinical readiness for discharge
• Leave written monitoring instructions daily for the incoming Day Support Specialist
• Train and competency-check Support Specialists on observation, documentation, and overdose response
• Review Support Specialist documentation for clinical accuracy
• Advise Intake on clinical suitability where a referral's needs may exceed the minimum-service model
• Coordinate with primary care providers, clinics, home health, hospitals, and pharmacies
• Make and document referrals
• Provide member education on condition management, medication, and follow-up care

Minimum Qualifications

• Current, unrestricted California RN license in good standing
• Two years of clinical nursing experience
• Current BLS certification
• Valid California driver's license and reliable transportation
• Sound independent clinical judgment; this role works without a nurse on-site alongside them

Preferred Qualifications

• Community health, public health, home health, correctional, shelter-based, or street medicine experience
• Wound care experience
• Experience with substance use, behavioral health, or homeless-serving programs
• Bilingual

Pre-Employment Requirements

• 18 years of age or older
• Satisfactory background check completed before unsupervised member contact
• Negative TB clearance obtained within the twelve months prior to start date
• Signed Staff Scope Acknowledgment
• Signed mandated reporter acknowledgment for elder and dependent adult abuse

Physical Requirements and Work Environment

• Work primarily on-site at the assigned Sprout Recuperative Care home
• Work in congregate residential settings serving adults with complex medical, behavioral health, and substance use needs
• Potential exposure to bloodborne pathogens and communicable disease; PPE provided and required

Schedule: Approximately 2 hours/day, plus participation in the paid after-hours on-call rotation.
