About Us

Sprout Organization runs Recuperative Care — a program funded through Medi-Cal (CalAIM Community Supports) for people who've just been discharged from the hospital but have nowhere safe to go to recover. Without a place like ours, many of these folks would end up back on the street, back in the ER, or back in the hospital within days. We give them a safe place to stay, three meals a day, and a team that's there around the clock while they get back on their feet.

Why This Job Matters

As a Recuperative Care Specialist, you're the person our members see the most. You're not just watching over the house — you're often the first one to notice that someone's struggling, the one who makes sure they get to their follow-up appointment, the one who keeps things calm when tension builds, and the one who makes a scary transition (hospital → nowhere to go → recovery) feel a little more human. This is real community support work. It's not clinical care — you're not a nurse — but it's the kind of steady, attentive presence that keeps people from falling through the cracks.

What You'll Actually Do

• Check in on every member during your shift — really check in, not just a walk-by — and notice if something seems off (pain, confusion, low mood, withdrawal)
• Walk the house on hourly rounds to make sure it's safe and calm
• Make sure meal times run smoothly; help make sure everyone gets fed and mealtime stays orderly
• Coordinate rides to doctor's appointments and help members stay on top of their follow-up care
• Welcome new members when they arrive and help them understand how the house works; support members as they prepare to move on
• Write clear, factual notes on what you observed and hand off anything important to the next shift
• De-escalate conflict calmly, and know exactly when to call 911, the supervisor, or the on-call nurse
• Help connect members to outside resources — housing help, community programs, follow-up care — by looping in the Supervisor or care team

Who We're Looking For

• Someone who genuinely likes people and wants to help them through a hard moment
• Calm, observant, and reliable — the kind of person others count on
• Comfortable working independently and communicating clearly, in person and in writing
• Respectful of people's privacy and dignity, no exceptions
• Experience in caregiving, recovery work, or homeless services is a plus — not required
• High school diploma or GED, and able to pass a background check and health screening
• CPR/First Aid certified, or willing to get certified

Good to Know

This is a non-clinical, community support role — you won't give medications, do wound care, or provide hands-on personal care. Members manage their own health needs; our nurse and outside providers handle anything medical. Your job is presence, attention, and follow-through: noticing, documenting, coordinating, and knowing when to bring in the right help.

Schedule

Shifts available: 7am–3pm, 3pm–11pm, 11pm–7am (some weekends/holidays required). Occasional in-person meetings/administrative tasks at Sprout's office in Gold River, CA may also be required.
