Position Summary

We are seeking a caring and dependable Licensed Practical Nurse (LPN) to join our clinical team. The LPN plays a key role in delivering high-quality nursing care by supporting providers, coordinating patient services, and addressing individual patient needs. This position is ideal for someone who is patient-focused, organized, and committed to serving the community.



Key Responsibilities

Patient Care


  • Evaluate patients to determine appropriate levels of care
  • Assist with patient care activities including taking vital signs and documenting chief health concerns
  • Prepare and administer medications, treatments, and injections as directed
  • Perform clinical tests and support patient examinations

Communication & Care Coordination


  • Contact pharmacies to order prescriptions
  • Facilitate follow-up appointments
  • Complete expanded assessments during initial or new patient encounters
  • Support medical investigations and treatment referrals
  • Help patients understand diagnoses, treatment plans, and next steps

Patient Education & Support


  • Provide education and written materials related to diagnoses and treatment plans
  • Assist patients and families in making informed healthcare decisions
  • Review treatment goals, resources, and available community supports


Required Qualifications

Education & Licensure


  • Graduate of an accredited school of nursing (required)
  • Current Licensed Practical Nurse (LPN) license in the State of Oklahoma (required)

Experience


  • 1–2 years of nursing experience in a community health center or hospital setting preferred

Knowledge, Skills & Abilities


  • Strong clinical knowledge and patient care skills
  • Good computer skills, including familiarity with medical documentation and office procedures
  • Strong organizational, time management, and self-motivation skills
  • Ability to maintain patient confidentiality and professionalism

Technology & Systems


  • Experience with Electronic Medical Records (EMR) systems preferred
  • Athena EMR experience a plus

Language Skills


  • Spanish-speaking / bilingual (English–Spanish) preferred
