Position Summary
We are seeking a caring and dependable Licensed Practical Nurse (LPN) to join our clinical team. The LPN plays a key role in delivering high-quality nursing care by supporting providers, coordinating patient services, and addressing individual patient needs. This position is ideal for someone who is patient-focused, organized, and committed to serving the community.
Key Responsibilities
Patient Care
- Evaluate patients to determine appropriate levels of care
- Assist with patient care activities including taking vital signs and documenting chief health concerns
- Prepare and administer medications, treatments, and injections as directed
- Perform clinical tests and support patient examinations
Communication & Care Coordination
- Contact pharmacies to order prescriptions
- Facilitate follow-up appointments
- Complete expanded assessments during initial or new patient encounters
- Support medical investigations and treatment referrals
- Help patients understand diagnoses, treatment plans, and next steps
Patient Education & Support
- Provide education and written materials related to diagnoses and treatment plans
- Assist patients and families in making informed healthcare decisions
- Review treatment goals, resources, and available community supports
Required Qualifications
Education & Licensure
- Graduate of an accredited school of nursing (required)
- Current Licensed Practical Nurse (LPN) license in the State of Oklahoma (required)
Experience
- 1–2 years of nursing experience in a community health center or hospital setting preferred
Knowledge, Skills & Abilities
- Strong clinical knowledge and patient care skills
- Good computer skills, including familiarity with medical documentation and office procedures
- Strong organizational, time management, and self-motivation skills
- Ability to maintain patient confidentiality and professionalism
Technology & Systems
- Experience with Electronic Medical Records (EMR) systems preferred
- Athena EMR experience a plus
Language Skills
- Spanish-speaking / bilingual (English–Spanish) preferred
