Description

Essential Duties and Responsibilities

  • Provide comprehensive nursing assessment and develop individualized plans of care.
  • Administer medications and treatments as prescribed.
  • Evaluate the patient’s response to care and update care plans accordingly.
  • Collaborate with physicians, social workers, chaplains, and other team members to provide coordinated, patient-centered care.
  • Educate patients, families, and caregivers regarding disease process, symptom management, and the hospice philosophy.
  • Document all care and communications in compliance with agency policy, state, and federal regulations.
  • Participate in interdisciplinary group (IDG) meetings and quality improvement initiatives.
  • Provide emotional support and guidance to patients and families throughout the end-of-life journey.
  • Ensure timely communication of changes in condition to the physician and team.
  • Participate in the on-call rotation to ensure 24/7 patient support.
  • Perform other duties as assigned to support the mission and values of Solace Hospice.

Qualifications

  • Current Registered Nurse license in the Commonwealth of Virginia.
  • Minimum of one (1) year of clinical nursing experience, preferably in hospice, home health, oncology, or medical-surgical settings.
  • Current CPR certification.
  • Excellent communication, assessment, and critical thinking skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Must possess reliable transportation, valid driver’s license, and current automobile insurance.

Physical Requirements

  • Must be able to lift, carry, and move patients or equipment weighing up to 50 pounds.
  • Requires frequent standing, walking, and bending.
  • Must be able to drive and work in various weather conditions, including rain, snow, ice, or other forms of inclement weather.

$38 - $43.5 per hour
