The Hospice Social Worker provides compassionate emotional support, advocacy, and resource coordination for patients and families facing life’s final chapter. In collaboration with the interdisciplinary team, the Social Worker helps ensure that each individual’s care is guided by dignity, choice, and peace. This position serves as a vital bridge between the clinical team, patients, families, and community resources. The Volunteer Services Coordinator oversees the recruitment, training, and coordination of hospice volunteers who provide compassionate companionship and support to patients, families, and the care team. Working closely with the interdisciplinary team, this role ensures that volunteer services are meaningfully integrated into patient care and meet the regulatory requirement that at least five percent of total hospice care hours are provided by volunteers.

Description

Essential Duties and Responsibilities

  • Conduct comprehensive psychosocial assessments of patients and families.
  • Identify emotional, financial, and social needs to develop individualized care plans.
  • Provide supportive counseling to patients, families, and caregivers throughout illness, death, and bereavement.
  • Facilitate access to community and financial resources, including Medicaid, advance directives, veteran benefits, and other support programs.
  • Collaborate with the interdisciplinary group (IDG) to address patient and family goals of care.
  • Participate in care conferences and assist in developing the plan of care.
  • Document all visits, communications, and interventions according to agency policy and regulatory requirements.
  • Provide education regarding coping, grief, and the hospice philosophy.
  • Participate in the on-call rotation as needed to support the team.
  • Perform other duties as assigned to support the mission and values of Solace Hospice
  • Recruit, screen, and train volunteers in accordance with state and federal hospice regulations.
  • Match volunteers with assignments based on patient and family needs, preferences, and volunteer skills.
  • Maintain documentation of volunteer activities, hours, and ongoing education.
  • Coordinate and monitor volunteer participation to ensure compliance with Medicare’s 5% volunteer requirement.
  • Provide ongoing supervision, support, and education to all active volunteers.
  • Plan and implement volunteer recognition and appreciation programs.
  • Collaborate closely with the Hospice Social Worker and interdisciplinary group (IDG) to ensure coordinated, holistic care.
  • Maintain volunteer records, confidentiality agreements, and performance evaluations.
  • Participate in community outreach and recruitment efforts to grow the volunteer base.
  • Perform other duties as assigned to support the mission and operations of Solace Hospice.

Qualifications

  • Bachelor’s degree in Social Work (BSW) required; Master’s degree in Social Work (MSW) preferred.
  • Licensure not required; hospice or healthcare experience strongly preferred.
  • Strong communication, assessment, and problem-solving skills.
  • Knowledge of community resources, social service systems, and end-of-life issues.
  • Ability to work both independently and collaboratively as part of the interdisciplinary team.
  • Reliable transportation, valid driver’s license, and current automobile insurance required.
  • Previous experience in volunteer coordination, hospice, or community engagement strongly preferred.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work independently and collaboratively with staff and volunteers.
  • Strong computer and record-keeping skills.

Physical Requirements

  • Must be able to lift, carry, or move materials weighing up to 25 pounds.
  • Requires frequent sitting, standing, and occasional travel to occasional driving to patient homes, facilities, volunteer sites or community events.
  • Must be able to drive and work in various weather conditions, including rain, snow, ice, or other forms of inclement weather.

$75,000 - $80,000 per year
