The Hospice Nurse Practitioner (NP) provides clinical expertise and compassionate support to patients and families in collaboration with the Hospice Medical Director and interdisciplinary group (IDG). The NP performs face-to-face encounters for hospice recertifications, provides palliative assessments, and supports symptom management to ensure the highest quality of life for patients under Solace Hospice’s care.

Description

Essential Duties and Responsibilities

  • Conduct face-to-face patient assessments for hospice eligibility and recertification in compliance with Medicare requirements.
  • Perform physical examinations, review medical histories, and document findings accurately and promptly.
  • Collaborate with the Hospice Medical Director, RN Case Managers, and IDG to develop and revise plans of care.
  • Evaluate and manage symptoms related to terminal illness, in collaboration with the Medical Director.
  • Provide patient and family education regarding disease progression, comfort measures, and care goals.
  • Communicate changes in condition or symptom control needs to the interdisciplinary team.
  • Participate in team meetings, quality improvement initiatives, and staff education as requested.
  • Serve as a resource for clinical staff in pain and symptom management best practices.
  • Provide compassionate, culturally sensitive care in patient homes, facilities, or inpatient settings.
  • Perform other duties as assigned to support the mission and values of Solace Hospice.

Qualifications

  • Licensed Nurse Practitioner in the Commonwealth of Virginia with current certification and prescriptive authority.
  • Minimum of two (2) years of clinical experience, preferably in hospice, palliative care, oncology, or geriatrics.
  • Current DEA registration (preferred) and CPR certification.
  • Strong clinical assessment and communication skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Reliable transportation, valid driver’s license, and current automobile insurance required.

Physical Requirements

  • Must be able to lift, move, and carry equipment weighing up to 25 pounds.
  • Requires frequent travel to patient homes and facilities.
  • Must be able to drive and work in various weather conditions, including rain, snow, ice, or other forms of inclement weather.

Salary

$60 - $75 per hour
