Type of Position: Full Time (40 hours per week)

Days: Monday-Friday

About Art Pharmacy

Art Pharmacy is an innovative healthcare organization that works with partners (in healthcare, education, and beyond) to provide social prescribing services to individuals to address various behavioral health symptoms and concerns. Art Pharmacy is focused on expanding and scaling social prescribing in the United States by, first, partnering with arts and cultural experiences within an individual's community and then working to provide access to these services directly to the individual. Art Pharmacy’s model is based on 40+ years of demonstrated impact that the arts & health field has on improving mental, emotional, and physical well-being and aims to provide access to these effective services to anyone and everyone who may benefit. 


Who are we looking for?

We are looking for an innovative, mission-driven, and creative mental healthcare professional to be a Care Navigator. This role is focused on providing direct services to the members we serve and supporting these members to utilize social prescribing services.


More About the Team and Who We Serve

Art Pharmacy’s Care Navigator team is responsible for ensuring Art Pharmacy’s program and services are delivered thoughtfully and in a timely manner to all members they support. Care Navigators are the main point of contact for Art Pharmacy members throughout their journey in the program. Care Navigators serve as the link between the individual and their arts and culture engagements. Care Navigators do not provide clinical care to members; however, team members tend to have a healthcare and/or clinical background.


They serve a variety of individuals in terms of ages, needs, and levels of support. The Care Navigator builds strong relationships with referred members to help them connect with health-improving arts and culture engagements. Care Navigators are committed to reducing the members’ barriers to the engagements by identifying obstacles with members, helping them navigate those obstacles, and using Art Pharmacy’s platform to recommend relevant engagements to increase the chance of uptake while also focusing on the best way to scale their services to serve as many individuals as possible. 



Things You Will Do:


Care Navigator Tasks Include:


  • Deliver high-quality, coordinated support to members throughout their journey in the program
  • This includes:
  • Using phone, email, video call, and text to interact and support members. These services include but are not limited to: 
  • Conducting outreach to enroll referred members
  • Completing intakes and reviewing member information
  • Offering arts & culture engagements through the Art Pharmacy platform
  • Answering questions and sharing more information about Art Pharmacy with current and prospective members
  • Booking and coordinating logistics for member participation in selected arts & culture engagements and providing relevant information/tickets to the member. 
  • Managing a caseload of members from entry to Art Pharmacy (referral) through intake assessment, regular arts engagements, midway assessments, and to the end of the program evaluation. 
  • Learning and catering services that are provided to each member per unique partnership. This includes partnerships with health payors, health systems, education systems, and beyond.

  • You are passionate about finding innovative and creative solutions to support individuals experiencing mental health concerns
  • You are an excellent listener and communicator (both spoken and written)
  • You are comfortable speaking with people you do not know on the phone and video chat, as well as via email and text
  • You are extremely organized and have great record-keeping skills 
  • You are highly proficient with utilizing computers, database management, and use of Google Suite
  • You are extremely flexible and able to quickly adapt to changes
  • You like taking the initiative in finding solutions to problems 
  • You enjoy thinking creatively in the face of obstacles
  • You show key skills to working with people including, but not limited to empathy, emotional resilience, and non-judgment
  • You have experience working with diverse populations (age, race/ethnicity, abilities, language, etc)
  • You are proactive in maintaining personal and professional learning and development and identifying your own training needs as they arise


Job Requirements and Qualifications

  • You have a bachelor’s degree in social work, psychology, sociology, etc.
  • You have healthcare experience and/or certification/license (LCSW or similar)
  • You have proven comfort with technology
  • You have reliable internet access
  • You have a quiet and confidential space to have calls with members


What Else You Should Know: 

  • This position is ideal for candidates looking for a less clinically focused role than traditional 1-1 care, while still supporting mental health access.
  • This position is fully remote 


Nice to Haves: 

  • Master’s Degree and/or active mental health licensure (LCSW, LMHC, etc.) is preferred 
  • You have experience in a start-up environment 
  • Art experience or interest
