Art Pharmacy is a tech-enabled healthcare company that works with health plans, health systems, universities, and community partners to address the mental health crisis through social prescribing. We partner with health plans (insurers) and providers (doctors) to enable the prescription of arts and culture programming for anxiety, depression, and loneliness.
Job Summary
Care Navigators - what we call Care Coordinator - are the main points of contact for Art Pharmacy members. The Care Navigator works with each referred member and Art Pharmacy’s proprietary software to create a personalized care plan, one focused on the specific needs of the member. Care Navigators are committed to reducing the members’ barriers to engagements by identifying obstacles with members, helping them navigate transportation issues, and promoting relevant arts and culture engagements.
The Care Navigator’s primary responsibility is ensuring organizational effectiveness by providing thoughtful delivery of Art Pharmacy’s day-to-day services. As the on-the-ground Art Pharmacy representative, the Care Navigator will also contribute to developing Art Pharmacy’s network of arts and culture partners. They help to build relationships with strategic partners and stakeholders, and they work with Art Pharmacy leadership to identify ways to improve our care and service. The Care Navigator is an essential part of the Art Pharmacy team; the role is designed for a caring individual with a growth mindset and the ability to manage several tasks at once, ranging from daily task work to big picture strategic thinking.
Duties & Responsibilities
The Care Navigator will:
- Work in collaboration with the Art Pharmacy team to deliver high-quality, coordinated support to empower members to access and engage with arts and culture engagements through the Art Pharmacy platform.
- Field inquiries from referred members via phone, email, and text to connect members with appropriate arts and culture engagements from the Art Pharmacy’s offerings.
- Book and coordinate logistics for member participation in selected arts engagements and provide relevant information/admission tickets to the member.
- Maintain up-to-date records of member activity and participation status.
- Manage a caseload of members from referral through intake assessment, regular arts engagements and midway assessments, to end of prescription evaluation.
- Maintain open communication with arts and culture partners to maintain up-to-date calendar offerings and relay attendee information as necessary.
- Participate in strategic relationship building initiatives with arts and culture and prescribing partners
- Contribute to the development of Art Pharmacy services and participate in support, supervision, and training as required.
- Ensure compliance with all relevant policies and procedures in relation to health and safety, equal opportunities, data protection, and confidentiality.
The role is a non-clinical role, however the care navigator will receive orientation and continued training to best be able to use social prescribing to empower people to take control of their health and wellbeing.
General Qualifications
- An existing resident of Massachusetts.
- Bilingual: English and Spanish
- Excellent listening and communication skills (both spoken and written) - must be comfortable speaking with strangers on the phone and video chat, as well as via email and text.
- Excellent organization and record keeping skills.
- Digital fluency — you know you’re way around a keyboard, file structure, and use of the internet.
- Proficient with database management and use of Google Suite
- Willingness to learn new technologies and incorporate use of new features (in propriety software).
- Takes initiative in finding solutions to problems and thinks creatively in the face of obstacles
- Demonstrates a practice of empathy, emotional resilience, and non-judgement. You care about people and want to help others with their health and well-being.
- Experience working with diverse populations (age, race/ethnicity, abilities, language, etc).
- Familiarity with arts & culture practices and their impact on health and wellbeing
- Proactive in maintaining personal and professional learning and development and identify own training needs as they arise.
