SECTION 2 | POSITION SUMMARY

The Assisted Living Caregiver is responsible for providing person-centered, compassionate care to residents of The Snowberry Small Type A Assisted Living and Memory Care Facility in accordance with each resident's individualized service plan (ISP) and all applicable Texas Health and Human Services Commission (HHSC) regulations. This position operates under the direction of the Administrator and works collaboratively with all care team members to ensure resident safety, dignity, and quality of life.



SECTION 3 | ESSENTIAL DUTIES AND RESPONSIBILITIES


Note:

A. Personal Care & Activities of Daily Living (ADLs)

●     Assist residents with bathing, grooming, dressing, oral hygiene, and toileting according to the ISP.

●     Provide assistance with mobility, transfers, and ambulation using proper body mechanics and assistive devices.

●     Assist with meal preparation, dining, and feeding as directed by the ISP; encourage nutritional intake.

●     Monitor and document intake/output, appetite changes, and any changes in resident condition.

B. Medication Assistance (as authorized under 26 TAC §553.261)

●     Assist residents with self-administration of medications as outlined in the ISP and in accordance with the facility's medication assistance policy.

●     Document medication assistance accurately in the Medication Assistance Record (MAR).

●     Report medication errors, refusals, or adverse reactions immediately to the supervisor.


Regulatory Note:

Texas Type A ALFs permit staff to assist with medication self-administration only. Administration of medications requires licensed nurse delegation per 22 TAC §225.

C. Resident Monitoring & Safety

●     Conduct and document resident wellness checks per facility policy and the resident's ISP.

●     Observe and promptly report to the supervisor any changes in physical, cognitive, or behavioral status.

●     Follow all emergency protocols, including fire evacuation procedures, elopement response, and emergency call system response.

●     Maintain a safe, hazard-free environment for all residents.

D. Documentation & Communication

●     Complete accurate, timely shift notes, incident reports, and other required documentation.

●     Participate in shift change handoff and communicate pertinent resident information to oncoming staff.

●     Maintain confidentiality of all resident information in compliance with HIPAA and state privacy laws.

●     Report suspected abuse, neglect, or exploitation immediately per facility policy and 26 TAC §553.41 (mandatory reporter obligations).

E. Housekeeping & Environmental Support

●     Maintain resident rooms in a clean, safe, and organized manner.

●     Perform light housekeeping duties including laundry, linen changes, and trash removal as assigned.

●     Report maintenance needs, safety hazards, or equipment malfunctions to the supervisor.

F. Activity & Engagement Support

●     Encourage and support resident participation in scheduled activities and social programs.

●     Assist with setting up and facilitating activities as directed.

●     Foster meaningful engagement that supports cognitive, emotional, and physical well-being.



SECTION 4 | REQUIRED QUALIFICATIONS

Education

●     High school diploma or GED equivalent required.

Must pass criminal history background check through the Texas Department of Public Safety (DPS) and HHSC Employee Misconduct Registry (EMR) prior to hire

Required prior to hire; no exceptions

Must not be listed on the HHSC Nurse Aide Abuse Registry or OIG Exclusions List

Verified at hire and annually

Current CPR / First Aid certification required or must obtain within 30 days of hire

Certificate copy maintained in personnel file

Must complete the HHSC-required 16-hour orientation training prior to providing unsupervised care

Must complete annual 12-hour in-service training

Must complete mandatory reporter training (abuse, neglect, exploitation) prior to working independently

Experience

●     Minimum six (6) months of experience in a caregiving, healthcare, or related human services setting preferred; will train the right candidate.


SECTION 5 | PREFERRED QUALIFICATIONS

●     Prior experience in assisted living, memory care, or skilled nursing setting.

●     Texas Certified Nurse Aide (CNA) certification.

●     Dementia care training or certification (e.g., Alzheimer's Association training).

●     Bilingual (English/Spanish).

●     Experience with person-centered care models.

Safe Handling Policy:

The Snowberry provides training in safe lifting techniques and makes mechanical lift equipment available. Employees must follow all safe handling procedures at all times.



SECTION 7 | WORKING CONDITIONS

●     Indoor environment in a residential assisted living facility.

●     May be required to work weekends, holidays, and overtime based on facility staffing needs.

●     Exposure to infectious illness and biological materials; PPE provided.

●     Fast-paced environment requiring the ability to respond calmly and effectively to emergencies.

●     Memory care assignments may involve working with residents who exhibit behavioral symptoms associated with dementia.
