Smart Rehab provides in-home therapy for older adults through two care models: Medicare Part B outpatient therapy and Medicare Part A home health therapy through partner agencies. Our Part B program is built around a simple idea, outpatient-quality care delivered in the patient’s home with one-on-one visits and a strong focus on mobility, balance, fall prevention, and home safety. Across both service lines, we’re known for therapist autonomy, clear communication, and a supportive, clinician-led culture, so clinicians can do great work and patients can stay safe and independent at home.

Smart Rehab partners with home health agencies to provide Medicare Part A speech therapy visits in patients’ homes. This role is designed for an SLP who enjoys a home health-style workflow, flexible volume, and community-based care.

Visits are completed based on agency referral needs and patient availability. Therapists work independently in the field while staying connected with the coordinating care team.

• Is comfortable communicating with an agency team and following established home health processes

Key Responsibilities

• Complete speech therapy evaluations, follow-up visits, reassessments, and discharges in the patient’s home
• Provide medically necessary, skilled speech therapy under the home health plan of care
• Evaluate and treat areas such as:
• swallowing/dysphagia and safe feeding strategies
• cognitive-communication (memory, attention, executive function)
• speech/language and functional communication needs
• voice as clinically appropriate
• Educate patients and caregivers on strategies to improve safety and function at home
• Communicate clinically relevant updates to the agency care team (as required)
• Complete documentation accurately and on time using the required EMR/platform
• Follow all home health, Medicare, and agency compliance expectations

Schedule & Coverage Area

• Flexible scheduling with visit volume based on referrals and your availability
• Coverage is based on geographic territory to reduce drive time
• Ability to accept additional visits as desired

Required Qualifications

• Licensed Speech Language Pathologist in the state of practice
• Home health experience preferred (but not required if comfortable working independently)
• Strong clinical decision-making and safety awareness in the home environment
• Valid driver’s license and reliable transportation
• Comfortable with electronic documentation and basic technology

Compensation & Pay

• Paid per visit as a 1099 independent contractor
• Competitive per-visit rates based on visit type and territory
