Smart Rehab partners with home health agencies to provide Medicare Part A speech therapy visits in patients’ homes. This role is designed for an SLP who enjoys a home health-style workflow, flexible volume, and community-based care.
Visits are completed based on agency referral needs and patient availability. Therapists work independently in the field while staying connected with the coordinating care team.

Key Responsibilities
- Complete speech therapy evaluations, follow-up visits, reassessments, and discharges in the patient’s home
- Provide medically necessary, skilled speech therapy under the home health plan of care
- Evaluate and treat areas such as:
- swallowing/dysphagia and safe feeding strategies
- cognitive-communication (memory, attention, executive function)
- speech/language and functional communication needs
- voice as clinically appropriate
- Educate patients and caregivers on strategies to improve safety and function at home
- Communicate clinically relevant updates to the agency care team (as required)
- Complete documentation accurately and on time using the required EMR/platform
- Follow all home health, Medicare, and agency compliance expectations
Schedule & Coverage Area
- Flexible scheduling with visit volume based on referrals and your availability
- Coverage is based on geographic territory to reduce drive time
- Ability to accept additional visits as desired
Required Qualifications
- Licensed Speech Language Pathologist in the state of practice
- Home health experience preferred (but not required if comfortable working independently)
- Strong clinical decision-making and safety awareness in the home environment
- Valid driver’s license and reliable transportation
- Comfortable with electronic documentation and basic technology

Compensation & Pay
- Paid per visit as a 1099 independent contractor
- Competitive per-visit rates based on visit type and territory
- Flexible volume depending on therapist availability
- Paid promptly on a regular schedule
