Smart Rehab is an outpatient occupational therapy practice that delivers care in the home for older adults. Occupational Therapists manage a local caseload, treat independently, and control their schedules. Care is relationship based and quality driven, not high volume or productivity based. Patients are not required to be home bound, and therapists are supported by a small, clinician led team.

Smart Rehab provides in-home therapy for older adults through two care models: Medicare Part B outpatient therapy and Medicare Part A home health therapy through partner agencies. Description

This position provides outpatient occupational therapy services in the home for older adults under Medicare Part B. Occupational Therapists deliver one-on-one care during 60-minute visits, focusing on functional independence, safety, activities of daily living, and quality of life. Therapists manage a local caseload, work independently, and maintain flexibility in scheduling. This is not a home health role, patients are not required to be homebound, and care is not productivity driven.

Key Responsibilities

  • Perform occupational therapy evaluations, follow-up visits, progress notes, and discharges in patients’ homes
  • Address ADLs including dressing, bathing, toileting, grooming, and meal preparation
  • Assess and address home safety, fall risk, and environmental barriers
  • Provide training in functional mobility, transfers, and compensatory strategies
  • Recommend and educate patients on adaptive equipment and home modifications
  • Develop and update plans of care in compliance with Medicare Part B requirements
  • Educate patients and caregivers to promote safety and independence
  • Complete documentation accurately and on time using the company EMR
  • Communicate with physicians and the Smart Rehab team as needed


Schedule & Caseload

  • Flexible schedule, therapists control when visits are completed
  • Local, geographic caseload to minimize drive time
  • Typical visits are 60 minutes, one-on-one
  • No productivity quotas, double booking, or clinic-style scheduling


Required Qualifications

  • Licensed Occupational Therapist in the state of practice
  • Eligible for Medicare Part B enrollment or reassignment
  • Experience working with older adults preferred
  • Valid driver’s license and reliable transportation
  • Comfortable working independently in the home setting
