Smart Rehab partners with home health agencies to provide Medicare Part A occupational therapy visits in patients’ homes. This role is ideal for an Occupational Therapist who enjoys a home health-style workflow, flexible volume, and community-based care. Visits are completed based on agency referral needs and patient availability. Therapists work independently in the field while staying connected with the coordinating care team.

Key Responsibilities
- Complete OT evaluations, follow-up visits, reassessments, and discharges in patients’ homes
- Provide medically necessary, skilled OT under the home health plan of care
- Address ADLs such as dressing, bathing, toileting, grooming, and meal prep
- Assess home safety and fall risk; identify environmental barriers to independence
- Provide caregiver training and patient education to support safe function at home
- Recommend and educate on adaptive equipment and basic home modification strategies
- Communicate clinically relevant updates to the agency care team (as required)
- Complete documentation accurately and on time using the required EMR/platform
- Follow all Medicare and agency compliance expectations
Schedule & Coverage Area
- Flexible scheduling with visit volume based on referrals and your availability
- Coverage is based on geographic territory to reduce drive time
- Ability to accept additional visits as desired
Required Qualifications
- Licensed Occupational Therapist in the state of practice
- Home health experience preferred (but not required if comfortable working independently)
- Strong clinical judgment and safety awareness in the home environment
- Valid driver’s license and reliable transportation
- Comfortable with electronic documentation and basic technology

Compensation & Pay
- Paid per visit as a 1099 independent contractor
- Competitive per-visit rates based on visit type and territory
