This is a Medicare Part A home health Certified Occupational Therapy Assistant (COTA) position. COTAs complete follow-up treatment visits in patients’ homes through Smart Rehab’s partner agencies, with flexible scheduling and visit volume based on referrals.

Smart Rehab provides in-home therapy for older adults through two care models: Medicare Part B outpatient therapy and Medicare Part A home health therapy through partner agencies. Description

Smart Rehab partners with home health agencies to provide Medicare Part A therapy visits in patients’ homes. This COTA role focuses on skilled treatment visits under a supervising Occupational Therapist’s plan of care, helping patients improve safety, independence, and function in their home environment.

Key Responsibilities

  • Complete skilled COTA follow-up visits in patients’ homes under a supervising OT’s plan of care
  • Provide ADL interventions as appropriate (e.g., dressing, bathing, toileting, grooming, meal prep)
  • Address functional mobility related to ADLs and safety within the home
  • Provide patient/caregiver education, compensatory strategies, and home program updates
  • Reinforce adaptive equipment use and basic home safety recommendations (per plan of care)
  • Communicate clinical changes, concerns, and progress to the supervising OT and agency team
  • Complete documentation accurately and on time using the required EMR/platform
  • Follow Medicare and agency compliance expectations


Schedule & Coverage Area

  • Flexible scheduling with visit volume based on referrals and your availability
  • Coverage includes the Greater St. Louis area (St. Louis, St. Charles, West County, and South County)
  • Ability to accept additional visits as desired


Required Qualifications

  • Licensed/Certified Occupational Therapy Assistant in the state of practice
  • Home health experience preferred (not required if comfortable working independently)
  • Valid driver’s license and reliable transportation
  • Comfortable with electronic documentation and basic technology

Compensation & Pay

Salary

$40 - $50 per hour
