Job Overview

We are seeking a compassionate and dedicated Licensed Graduate Social Worker to join our mental health and behavioral health team. This role offers an opportunity to provide comprehensive patient care, conduct assessments, and deliver various therapeutic services within a supportive clinical environment. The ideal candidate will possess strong skills in behavioral therapy, applied behavior analysis, and clinical counseling, with a focus on enhancing patient well-being across diverse populations, including geriatrics and individuals facing aging issues. This position emphasizes adherence to HIPAA regulations and meticulous medical documentation to ensure confidentiality and quality care.

Duties

• Conduct thorough patient assessments and diagnostic evaluations to inform treatment planning
• Provide individual and group therapy sessions utilizing evidence-based practices such as psychodynamic therapy, Christian counseling, and behavioral health techniques
• Develop and implement treatment plans aligned with patient needs, including home & community care strategies
• Manage intake processes, ensuring smooth onboarding of new patients while maintaining accurate medical records in compliance with HIPAA standards
• Deliver specialized counseling services addressing aging issues, geriatrics, and mental health concerns
• Engage in case management activities to coordinate care across multidisciplinary teams
• Document all clinical interactions accurately within medical documentation systems
• Maintain confidentiality of patient information in accordance with HIPAA regulations
• Participate in ongoing professional development to stay current with best practices in social work and behavioral health

Requirements

• Valid licensure as a Graduate Social Worker (LGSW or equivalent) with eligibility for full licensure as a Licensed Graduate Social Worker
• Experience or training in behavioral therapy, applied behavior analysis, and mental health counseling techniques
• Strong understanding of medical terminology, medical records management, and HIPAA compliance
• Proven ability to perform patient care tasks including intake assessments and diagnostic evaluations
• Familiarity with clinical counseling modalities such as psychodynamic therapy, Christian counseling, and group therapy settings
• Experience working with diverse populations, including geriatrics and individuals facing aging issues or complex mental health challenges
• Home & community care experience is preferred but not required
• Excellent communication skills with the ability to build rapport with patients across age groups and backgrounds
• Ability to work independently while collaborating effectively within a multidisciplinary team
