The Lead Nurse Coordinator for the Day Habilitation Program will oversee and manage health-related workshops and training for individuals participating in the program. This role is key in ensuring that participants receive high-quality, safe, and effective health education and care, as well as fostering collaboration with a multidisciplinary team to meet the unique needs of each individual. The coordinator will also manage the planning, delivery, and assessment of workshops that enhance the physical and emotional well-being of program participants.

Key Responsibilities:


  • Workshop Coordination:
  • Plan, organize, and manage workshops and health-related educational sessions tailored to the needs of individuals in the Day Habilitation Program.
  • Develop and maintain a yearly schedule for workshops, ensuring alignment with program goals and participant needs.
  • Collaborate with program staff, medical professionals, and external experts to deliver content that is relevant, engaging, and beneficial.
  • Health Education:
  • Provide health education on topics such as nutrition, medication management, personal care, and other relevant wellness topics.
  • Tailor educational materials to suit the learning abilities and preferences of participants.
  • Assist in the development of accessible and inclusive workshop formats, ensuring participants with diverse needs can benefit.
  • Participant Support:
  • Assess and monitor the health and well-being of program participants, ensuring workshops are meeting their needs.
  • Offer personalized guidance and recommendations for individual participants where appropriate.
  • Serve as a resource for program staff on health-related questions and concerns, and facilitate referrals to healthcare providers as needed.
  • Collaboration & Teamwork:
  • Work closely with interdisciplinary teams, including social workers, occupational therapists, and behavioral health specialists, to integrate health education into the broader scope of participant development.
  • Provide support to program staff in managing the physical and emotional needs of participants during workshops.
  • Facilitate communication between participants, families, and healthcare providers as necessary to ensure continuity of care.
  • Documentation & Reporting:
  • Maintain accurate and up-to-date records of workshop content, attendance, and participant progress.
  • Report on workshop outcomes and feedback to program management to ensure continuous improvement and effectiveness.
  • Ensure compliance with regulatory standards, including health and safety protocols.
  • Continuous Improvement:
  • Stay updated on current health trends, best practices, and research relevant to Day Habilitation programs.
  • Continuously evaluate and improve the quality of health-related workshops based on participant feedback, industry standards, and program needs.


Qualifications:

  • Education:
  • Bachelor’s degree in Nursing (BSN) required.
  • Valid Registered Nurse (RN) license in Ohio.
  • Experience:
  • Minimum of 2 years of nursing experience, preferably in a community or rehabilitative healthcare setting.
  • Experience coordinating or leading workshops, educational sessions, or training programs is highly desirable.
  • Experience working with individuals with disabilities or in a Day Habilitation or similar setting preferred.
  • Skills & Abilities:
  • Strong communication and interpersonal skills, with the ability to engage and educate diverse audiences.
  • Ability to assess, plan, and deliver workshops that are accessible and relevant to participants.
  • Knowledge of health and wellness topics, particularly as they relate to individuals with disabilities or special needs.
  • Strong organizational and time-management skills.
  • Proficiency with Microsoft Office Suite and other relevant software tools.
