Job Description:
1. Understand the key values and activities of case management in a mental health setting.
2. Demonstrate understanding of mental illness and the treatment process.
3. Understand the co-occurring illness process and how it impacts individuals.
4. Assist program participants in accessing and maintaining resources for independent community living.
5. Collaborate with clients to create a service and rehabilitation plan that meets their needs.
6. Provide ongoing support and monitoring to ensure services are effective.
7. Conduct individual sessions to help clients address challenges and set goals.
8. Advocate for clients to access necessary services and support.
9. Coordinate with families to involve them in the rehabilitation process.
10. Evaluate services to make improvements and enhance outcomes.
11. Maintain regular contact with program participants throughout their service journey.
12. Coordinate access to housing, social services, and other community resources.
13. Assist clients in navigating financial entitlements for income and healthcare coverage.
14. Uphold client rights and advocate on their behalf.
15. Facilitate access to vocational rehabilitation services.
16. Develop supports for employed program participants.
17. Assist clients with daily living skills and independence.
18. Monitor and assess client behavior and functioning to support treatment planning.
19. Connect clients with community services and resources.
20. Help clients learn appropriate social skills and behaviors.
21. Assist clients in maintaining connections with family members and support systems.
22. Collaborate with multidisciplinary teams to create individualized care plans.
23. Conduct functional assessments to inform service planning.
24. Participate in organizational processes such as quality assurance and health/safety activities.
25. Conduct regular check-ins with assigned caseload contacts.
26. Facilitate individual, family, and group therapy sessions.
Level of Education/Training: Master’s Degree in a health-related human
service field. LPC, LMSW or LCSW preferred.
