The Role

As a Nurse Case Manager, you will oversee patient care plans and coordinate services to meet the needs of individuals facing various health challenges. You will work closely with patients, families, and healthcare providers to ensure that care is delivered effectively and efficiently.

What You'll Do

- Assess patient needs and develop individualized care plans.

- Coordinate with healthcare providers to facilitate access to necessary services.

- Monitor patient progress and adjust care plans as needed.

- Provide education and support to patients and their families regarding treatment options and resources.

- Advocate for patients to ensure they receive appropriate care and support.

- Document all interactions and maintain accurate patient records.

- Collaborate with a multidisciplinary team to enhance patient outcomes.

What We're Looking For

- Active nursing license in the state of practice.

- Experience in case management or a related field.

- Strong communication and interpersonal skills.

- Ability to work independently and as part of a team.

- Proficiency in electronic health records and case management software.

Nice to Have

- Certification in case management or a related specialty.

- Experience in mental health or behavioral health settings.

- Bilingual skills are a plus.
