GRANT-FUNDED PROGRAM STAFF • 1 ROLE


Essential responsibilities

  • Complete timely, respectful intake and strengths-based needs assessments for program participants.
  • Develop individualized reentry plans addressing behavioral health, employment, housing, education, transportation, healthcare, and other stabilization needs.
  • Coordinate warm referrals and follow through with participants and partner providers to confirm service connection.
  • Maintain participant contact for three to six months using flexible, trauma-informed engagement strategies.
  • Collaborate with Circle Keepers and RRB staff before and after Accountability and Welcome Home Circles.
  • Document contacts, referrals, milestones, barriers, and outcomes accurately in MyRJ and approved tracking tools.
  • Protect confidentiality, follow mandatory-reporting requirements, and maintain appropriate professional boundaries.


Minimum qualifications

  • Relevant experience in reentry, case management, social services, behavioral health, violence prevention, or community-based navigation.
  • Demonstrated ability to build trust with justice-impacted adults and work across cultures without judgment.
  • Strong organization, documentation, communication, and follow-through skills; comfort using case-management technology.
  • Knowledge of Baltimore-area resources and ability to travel locally. Lived experience with reentry is valued.
