RaisingHealth Partners (formerly known as Academy of Medical & Public Health Services (AMPHS)) is a Brooklyn-based, nonprofit organization dedicated to eliminating barriers to accessing healthcare for immigrants and other under-served communities in NYC. Every year, we proudly serve and connect more than 20,000 New Yorkers to healthcare systems. Our programs equip immigrant populations with the knowledge to help them navigate the complexities of living in a new city and transition into post-secondary education and/or employment opportunities. At the same time, we also educate community members on their immigrant rights, healthy living, nutrition and health access, and create space for civic engagement.

Due to expansion and growth, we are immediately looking for a Community Health Partner (CHP) professionals to provide education, case management support, and outreach across RaisingHealth's health, social service, and education programs, creating a streamlined care model across programs. The CHP help families understand and access community services and resources, while guiding them to adopt healthy behaviors.


Responsibilities include but are not limited to:

Providing culturally sensitive outreach and education on public health, social justice, immigrant and healthcare rights, and public benefits topics and for RaisingHealth Partners health and immigrant service programs

  • Providing interpretation, follow-up, and case management for health and social services
  • Enhancing civic engagement through awareness-building programming, presentations, and
  • Developing and maintaining relationships with neighborhood stakeholders
  • Building rapport with community members and provide ongoing follow-up, support, and advocacy.


Minimum Qualifications:


  • High School Diploma required. Bachelor’s degree highly preferred.
  • Two (2) years of proven work experience as a Community Health Worker/Partner or similar role.
  • Prior case management, outreach, canvassing, or organizing experience.
  • Excellent communication skills, including the ability to listen well, speak articulately in a public setting and use language appropriately.
  • Bilingual language skills required. RaisingHealth has several CHP roles that require one of the following:
  • Spanish/English skills or Chinese/English skills
  • Knowledge of some medical terminology preferred, but not required.
  • Basic computer skills and knowledge in Microsoft Office.
  • Ability to work a full time schedule - Tuesday through Saturday from 10am to 6pm
