Compensation: $25 - $30 per hour

We are looking for an experienced Lead Case Manager to support our Section 13 Targeted Case Management program. This position combines direct work with children and families with staff mentorship, care coordination, and support for the program’s daily operations.

We’re looking for someone who can build trust with families, help coworkers navigate challenges, and keep the details moving without losing sight of the people behind them.

What you’ll do

  • Maintain an assigned caseload and help children and families access medical, behavioral health, educational, social, and community resources.
  • Complete comprehensive assessments and develop individualized service plans with members, caregivers, and their chosen supports.
  • Coordinate child and family team meetings, referrals, service transitions, and follow-up.
  • Advocate for member choice and help address barriers such as transportation, waitlists, and difficulty navigating service systems.
  • Support new case managers through onboarding, practical guidance, and ongoing mentorship.
  • Help monitor assessment and service-plan deadlines, documentation completion, required signatures, and follow-through on referrals.
  • Assist with chart reviews and quality improvement in collaboration with the program’s qualified supervisor.
  • Work closely with agency leadership, clinicians, schools, healthcare providers, and community partners.
  • Maintain timely, accurate documentation in Alleva and follow MaineCare Section 13 requirements and agency policies.
  • Bring concerns about safety, service gaps, or staff support needs to the appropriate supervisor.

Qualifications

  • Meet MaineCare Section 13 case manager qualifications, including a relevant bachelor’s degree; a qualifying master’s degree; or a bachelor’s degree in an unrelated field with at least one year of relevant full-time-equivalent human services experience.
  • Experience providing targeted case management to children and families; MaineCare Section 13 experience strongly preferred.
  • Previous experience mentoring, training, or leading staff preferred.
  • Strong knowledge of Maine community resources and the systems families navigate.
  • Clear communication, strong organization, and dependable follow-through.
  • Ability to work independently, collaborate with a team, and maintain professional boundaries and confidentiality.
  • Commitment to trauma-informed, culturally responsive, strengths-based care.
  • Ability to travel to homes, schools, and community settings within the assigned service area.
  • Ability to complete required background checks and program training, including Department-approved wraparound planning training.
