Rae of Hope, LLC is seeking a compassionate and organized Targeted Case Manager (TCM) to support children and families in navigating behavioral health services and community resources. This role focuses on helping families access the supports they need, coordinating care among providers, and ensuring children receive services that promote stability, safety, and long term well being. As a Targeted Case Manager, you will work directly with children and their caregivers to coordinate behavioral health and community based services. You will play a key role in helping families identify needs, connect with resources, and build a strong support network.
Responsibilities include:
• Conduct comprehensive assessments to identify strengths, needs, and service goals
• Develop and maintain individualized service plans with families
• Coordinate care among providers, schools, medical professionals, and community partners
• Advocate for children and families to ensure access to appropriate services and supports
• Help families navigate systems such as behavioral health services, education supports, and community programs
• Maintain accurate and timely documentation in accordance with MaineCare standards
• Monitor progress and adjust service plans as needed
• Participate in team meetings and clinical supervision as required

Candidates must meet MaineCare Targeted Case Management qualifications, including:
• Bachelor’s degree in social work, psychology, counseling, education, human services, or a related field
OR
• Bachelor’s degree in another field with relevant experience working with children or families
Preferred qualifications:
• Experience in behavioral health, child welfare, education, or community services
• Knowledge of MaineCare services and community resources
• Experience working with children and families facing emotional or behavioral challenges
• Valid driver’s license and reliable transportation
• Ability to pass required background checks
• Strong organizational and documentation skills
