Position Overview
Prevounce Health is seeking an innovative, results-driven Director of Care Management to lead and scale our growing remote care management operations.
This leadership position is ideal for someone who thrives in a fast-paced, technology-driven healthcare environment and knows how to turn operational data into measurable results.
Reporting to the VP of Operations, the Director of Care Management will oversee the performance, efficiency, and continued development of our remote care management department. This individual will be responsible for driving patient engagement, improving team productivity, implementing scalable workflows, and leveraging technology and automation to support organizational growth.
This is an operations-focused leadership position within a technology-enabled healthcare organization, rather than a traditional bedside or facility-based care management role.
The ideal candidate brings experience in HealthTech, telehealth, virtual care operations, or high-volume healthcare contact centers, along with a demonstrated ability to lead remote teams, improve operational performance, and implement innovative solutions.

- Utilize real-time reporting, operational dashboards, and performance analytics to identify trends, address inefficiencies, and drive measurable improvements.
- Oversee high-volume patient outreach workflows, including call queues, outreach cadence, response times, and workforce utilization.

- Champion the adoption of emerging healthcare technologies, AI-enabled tools, automation, and digital solutions to improve care management operations.
- Identify opportunities to streamline patient outreach, documentation, reporting, and administrative processes through technology.
- Partner with internal technology, product, and operations teams to improve software functionality, workflow integration, and user experience.
- Lead process improvement initiatives that increase productivity, reduce manual work, and support scalable growth.
- Evaluate and implement new operational tools, reporting solutions, and technology-enabled workflows.
- Support rapid testing, implementation, and refinement of new processes based on operational results and business needs.
Patient Engagement & Care Delivery
- Oversee patient engagement strategies to improve outreach effectiveness, program participation, and long-term retention.

- Balance operational efficiency with patient satisfaction, quality, and continuity of care.
Department Growth & Operational Strategy
- Develop scalable processes, organizational structures, and performance management systems to support continued growth.
- Identify opportunities to improve patient engagement, operational capacity, service quality, and financial performance.
