Position Overview: Seeking a dedicated and compassionate Nurse Advocate to join our team. As a Nurse Advocate, you will play a crucial role in providing community case management support to our clients and ensuring they receive the highest level of care. You will serve as a liaison between patients, their families, and healthcare providers, addressing concerns and promoting effective communication and collaboration.

Responsibilities:

1. Medical Advocacy: Serve as a strong advocate for clients and their families.
2. Care Coordination: Assuring clients' needs are addressed.

Qualifications:

1. Education: Possess a valid Registered Nursing license and hold a BSN nursing degree.
2. Experience: Prior experience as a case manager or in a similar role is preferred. Minimum 3 years clinical experience in a healthcare setting is required.
3. Communication skills: Excellent verbal and written communication skills, with the ability to effectively communicate complex medical information to clients and their families.
4. Empathy and Compassion: Demonstrate a deep level of empathy, compassion, and understanding towards patients and their families.
5. Critical Thinking: Strong ability to assess situations, identify potential issues, and develop appropriate solutions.
6. Interpersonal Skills: Good interpersonal skills to build rapport and establish trust.
