Job Posting: Weekend Caregiver (HPC) — Miamisburg, OH

Position Classification / Working Title: Homemaker Personal Care (HPC) — Direct Care Staff

Location: Miamisburg, Ohio (client’s home / family environment)

Schedule: Saturdays & Sundays — 8:30 AM to 5:00 PM

Employment Type: Hourly | Non-Exempt (Overtime Eligible)

Supervised By: Zontaye Richardson / Jared Wade

About the Role

On the Horizon, LLC is seeking a passionate, reliable Weekend Caregiver to provide Homemaker Personal Care (HPC) services for an individual with a disability in Miamisburg, OH. The client has limited verbal communication but uses an iPad to communicate. This role requires someone who is patient, attentive, and comfortable working in a home where family members may be present throughout the day.

Client Support Needs

  • Assistance with feeding/meals
  • Assistance with restroom/toileting routines
  • Engagement and supervision throughout the day
  • Support with mobility needs (client does not walk but can stand with staff assistance)

Key Responsibilities (Essential Job Duties)

  • Plan, coordinate, and supervise daily activities, including social and leisure experiences, while ensuring the individual’s safety and well-being at all times
  • Support basic life activities as indicated in the Individual Service Plan (ISP)
  • Report and document the individual’s progression/regression and any notable changes
  • Maintain accurate records and documentation in accordance with agency and regulatory requirements
  • Attend staff meetings and successfully complete all required trainings
  • Build and maintain positive relationships with individuals served, families, guardians, and the community
  • Be prepared to respond to emergencies and communicate concerns/incidents to your supervisor
  • Ensure individual rights are protected and supported
  • Follow all OTH policies and procedures and all safety standards required by regulatory agencies
  • Assist individuals in advocating for their needs
  • Perform other related duties as assigned to enhance quality of life

Requirements & Qualifications

Minimum Qualifications:

  • High School Diploma or GED
  • Must be 18 years of age or older
  • Ability to deliver Homemaker Personal Care services in accordance with applicable regulations
  • Reliable transportation


Role-Specific Requirements:

  • Must be able to commit to Saturdays and Sundays consistently
  • Must be able to lift up to 80 pounds
  • Strong communication skills and the ability to work respectfully in a family home environment


Pre-Employment Requirements:

  • Ability to pass a background check and drug screening
