Position Summary

The Telephone Triage Nurse is a vital member of Octiva Healthcare's clinical team. In this role, you will practice professional nursing through telephonic assessment, utilizing your clinical expertise, physician guidance, and nationally recognized telephone triage protocols.

You will evaluate patient symptoms, determine the appropriate level of care, provide nursing education and self-care recommendations, coordinate prescription refill requests, and collaborate with healthcare providers to ensure safe, high-quality patient care.

All responsibilities are performed within departmental policies, organizational standards, applicable state regulations, and the scope of practice established by the Board of Registered Nursing.


Essential Responsibilities

  • Perform comprehensive telephone assessments.
  • Evaluate patient symptoms and determine the appropriate level of care, including:
  • Home care
  • Primary care follow-up
  • Same-day appointments
  • Urgent Care
  • Emergency Department referral
  • Transfer to an Octiva Healthcare provider when appropriate
  • Provide evidence-based nursing advice and patient education.
  • Educate patients regarding medications, symptom management, warning signs, and follow-up recommendations.
  • Coordinate medication refill requests according to organizational policies.
  • Collaborate with physicians, nurse practitioners, clinic staff, and internal clinical teams to ensure continuity of care.
  • Escalate urgent and emergent clinical situations according to established workflows.
  • Review patient histories within multiple Electronic Medical Record (EMR) systems.
  • Maintain accurate, timely, and detailed clinical documentation.
  • Deliver compassionate, patient-centered care while maintaining exceptional customer service.
  • Perform additional duties as assigned.

Qualifications

Education

  • Graduate of an accredited School of Nursing (Required)
  • Bachelor of Science in Nursing (BSN) preferred

Required Licensure

Applicants must meet all of the following requirements:


  • Active California Registered Nurse License
  • Active Compact (Multi-State) Registered Nurse License
  • Active, unrestricted RN licenses in good standing

Experience

  • Minimum of three (3) years of recent Registered Nurse clinical experience required.
  • Previous telephone triage or telehealth nursing experience strongly preferred.
  • Experience in one or more of the following areas is highly preferred:
  • Emergency Department
  • Med-Surg
  • Urgent Care
  • Family Medicine
  • Internal Medicine
  • Telephone Triage

Knowledge & Skills

  • Fluent in both English and Spanish (written and verbal) required
  • Strong clinical assessment and critical thinking skills
  • Excellent telephone communication skills
  • Ability to make sound clinical decisions in a fast-paced environment
  • Strong understanding of symptom assessment and urgent/emergent care criteria
  • Proficiency with multiple Electronic Medical Record (EMR) systems
  • Ability to prioritize multiple clinical situations effectively
  • Strong attention to detail and documentation accuracy
  • Ability to work independently while collaborating with a multidisciplinary team
  • Commitment to providing compassionate, patient-centered care

Technical Requirements

  • Dedicated, quiet home office free from distractions
  • Dual monitors preferred
  • Comfortable navigating multiple software platforms simultaneously


To ensure reliable performance, security compliance, and compatibility with Octiva's clinical tools, all nurses working remotely must meet the following minimum computer specifications:

Operating System

Windows 11 (Home or Pro) — Recommended

macOS Ventura (13) or newer — Accepted

Linux (stable, LTS release — e.g. Ubuntu 22.04/24.04) — Accepted

Chromebooks and Chrome OS are not supported

Android, iPadOS, or any other mobile/tablet OS is not supported

Processor (CPU)

Intel Core i5 (8th Gen or newer) — Minimum

AMD Ryzen 5 (3000 series or newer) — Minimum

Apple Silicon (M1/M2/M3) meets this requirement automatically

Older or low-power processors (Intel Celeron, Pentium, Atom, or equivalent) are not recommended and may result in performance issues.

Memory (RAM)

8 GB minimum is required to run Citrix, Nextiva, Teams, browser tabs, and email simultaneously without performance degradation

16 GB recommended for optimal multitasking

Storage

256 GB SSD minimum (HDD is not recommended due to performance impact)

Adequate free space is required — a nearly full drive can cause application crashes and system instability

Internet Connection

Wired (Ethernet) connection required

⚠️ Wi-Fi is not recommended — wireless connections introduce latency and instability that can affect VoIP call quality (Nextiva) and Citrix session reliability

Minimum recommended speed: 25 Mbps download / 10 Mbps upload

Additional Requirements

Built-in or external webcam and microphone (for Teams meetings)

Up-to-date browser (Chrome or Edge recommended)
