Key Responsibilities -Deliver direct, person-centered clinical care to residents requiring long-term care or post-acute physical rehabilitation. -Perform comprehensive clinical assessments, monitor vital signs, and maintain clear, accurate electronic medical documentation. -Administer medications, intravenous therapies, and specialized wound care in accordance with physician orders and state standards. -Supervise LPNs and CNAs on assigned shifts, ensuring smooth unit workflow and high standards of safety. -Collaborate with interdisciplinary healthcare teams (physicians, physical therapists, social workers) to continuously update care plans. Qualifications -Active, unencumbered NY State Registered Nurse (RN) License. -Current Basic Life Support (BLS) / CPR Certification. -Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN). -Experience in long-term care, sub-acute rehab, or acute clinical settings preferred (New graduates welcome to apply). -Strong clinical judgement, communication, and interpersonal skills.
