Job Overview

We are seeking a compassionate and skilled Registered Nurse for Home care to provide high-quality medical and personal care to pediatric and teenage patients in their homes. The ideal candidate will possess a strong nursing background and the ability to deliver comprehensive, individualized care to young patients with complex medical needs, including tracheotomy management and complete support for daily living activities. This role is vital in ensuring the health, well-being, and comfort of these patients and their families.

Duties and responsibilities:

  • Conduct comprehensive patient assessments, including vital signs and medical history, to evaluate health needs and monitor progress.
  • Develop and implement individualized care plans tailored to the unique needs of pediatric and teenage patients.
  • Administer medications and treatments as prescribed by physicians, ensuring adherence to prescribed regimens.
  • Provide hands-on assistance with bathing, feeding, and other daily living activities to support patients' overall health and hygiene.
  • Monitor and care for patients with tracheotomies, including cleaning and maintaining the tracheotomy site, suctioning, and addressing complications as needed.
  • Observe patients' conditions, noting any changes or concerns, and promptly report them to the appropriate healthcare professionals.
  • Collaborate with interdisciplinary teams to coordinate comprehensive patient care effectively.
  • Educate patients' families about health management, tracheotomy care, and other critical aspects of caregiving.
  • Maintain accurate and thorough medical records, ensuring compliance with medical health record standards and regulatory requirements.
  • Ensure a safe environment for patients by following infection control protocols and safety guidelines.

Qualifications

  • Licensure: Valid nursing license in the state of practice or multi-state Compact license.
  • Experience: Prior experience in home care, pediatric care, or similar settings preferred.

Skills:

  • Proficiency in vital signs monitoring and administering prescribed treatments.
  • Expertise in tracheotomy care and managing medically fragile patients.
  • Strong documentation skills to maintain detailed and compliant patient health records.
  • Exceptional communication skills to interact effectively with patients, families, and healthcare teams.

Additional Qualifications

  • Commute Flexibility: Willing to commute up to 40 minutes from home to the assigned patient's location, ensuring timely and reliable care delivery.
  • Weekend Availability: Open to working 4 out of 8 weekend days per month (e.g., alternating weekends, every Saturday, or every Sunday for day or night shifts).
  • Transportation Requirements: Must possess a valid driver’s license and reliable vehicle. Flexibility to care for more than one patient during shifts, if required. Reimbursement provided for any travel expenses from the first patient to the last patient.
  • Client Adaptability: Comfortable working with diverse patient demographics, including male/female clients, geriatric, or pediatric populations, based on assignment needs.
  • Start Date Readiness: Available to begin work within 3 to 6 weeks of hire to meet patient care demands.

Hourly Rate: $39 - $41

Expected hours: 36 to 40 per week
