Job Overview

We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will possess a strong background in patient assessment and primary care, with experience in IV infusion and familiarity with medical terminology. This role requires excellent communication skills, the ability to work independently, and a commitment to delivering patient-centered care.

Duties

  • Conduct thorough patient assessments to determine individual care needs and develop personalized care plans.
  • Administer IV infusions and manage medication schedules as prescribed by physicians.
  • Provide education and support to patients and their families regarding health management, including family planning and memory care.
  • Collaborate with healthcare teams, including clinics and teaching hospitals, to ensure continuity of care.
  • Utilize electronic health records systems such as Cerner to document patient information accurately.
  • Assist with durable medical equipment (DME) setup and training for patients as needed.
  • Monitor patient progress and adjust care plans accordingly, maintaining open communication with physicians and family members.

Requirements

  • Valid nursing license in the state of practice.
  • Proven experience in home health care or a related field is preferred.
  • Strong knowledge of medical terminology and patient assessment techniques.
  • Experience with IV infusion procedures is highly desirable.
  • Familiarity with electronic health records systems, particularly Cerner, is a plus.
  • Ability to work independently while managing multiple patient needs effectively.
  • Excellent interpersonal skills for effective communication with patients, families, and healthcare professionals.
  • Commitment to providing compassionate, high-quality care tailored to each patient's unique situation.


Job Types: Full-time, Contract

Hourly Rate: $33 - $35

Expected hours: 36 to 40 per week
