Position Summary:

We are seeking a Home Health Registered Nurse (RN) to provide per-visit patient care in Fauquier County. This role includes case management, OASIS documentation, and using an EMR system for all charting. If you are experienced, organized, and passionate about home health, this is a great opportunity to work with a supportive team while enjoying flexibility and autonomy in your schedule.

Key Responsibilities:

• Conduct skilled nursing visits in patients’ homes.
• Perform Start of Care (SOC), Recertifications, Resumptions of Care (ROC), and Discharges using OASIS.
• Develop, coordinate, and oversee personalized care plans as part of case management duties.
• Communicate effectively with physicians, therapists, and interdisciplinary teams.
• Educate patients and families on disease management and health maintenance.
• Complete timely and accurate documentation using an Electronic Medical Record (EMR) system.
• Ensure compliance with agency policies, state regulations, and best practices in home health care.

Qualifications:

• Current and unrestricted Registered Nurse (RN) license in Virginia.
• Minimum of 1 year of recent Home Health experience.
• Proficient in OASIS documentation and skilled in using EMR systems (e.g., Kinnser, WellSky, Axxess, etc.).
• Strong communication, assessment, and critical-thinking skills.
• Reliable transportation and a valid driver’s license..
• Must be comfortable traveling within the assigned coverage areas.
