Home Health Physical Therapy
The Physical Therapist is responsible for evaluating, planning, and implementing individualized physical therapy treatment programs for patients in the home health setting. The Physical Therapist works collaboratively with physicians, nurses, therapists, caregivers, and interdisciplinary team members to improve patient mobility, strength, balance, safety, and overall functional independence.
Perform comprehensive physical therapy evaluations and reassessments in accordance with physician orders and agency policies.
Develop individualized treatment plans and establish measurable patient goals.
Provide therapeutic exercises, gait training, balance training, transfer training, and mobility interventions.
Assess and recommend durable medical equipment and assistive devices.
Educate patients and caregivers regarding exercises, fall prevention, home safety, and mobility techniques.
Monitor patient progress and modify treatment plans as appropriate.
$85 - $100 per hour
