The Intake Specialist is responsible for assisting eligible individuals in accessing housing-related services through the CalAIM Community Supports program. This role focuses on collecting required documentation, assessing eligibility, and submitting complete service authorization requests to Managed Care Plans (MCPs). The Intake Specialist works closely with clients, internal staff, and health plan representatives to ensure timely and accurate processing of requests for housing transition, tenancy sustaining, and housing navigation services.

Description

Key Responsibilities:

  • Conduct intake interviews with clients referred for Community Supports under CalAIM.
  • Gather and review required documentation, including identification, proof of income, medical necessity forms, and consent forms.
  • Communicate with health plans to follow up on submitted requests and resolve any issues related to missing information or documentation.
  • Collaborate with Navigators and other departments to ensure clients are connected to services upon approval.
  • Maintain accurate and timely documentation in compliance with HIPAA and organizational guidelines.
  • Attend trainings related to CalAIM, housing supports, and systems used for referrals and submissions.
  • Provide excellent customer service and support to individuals with complex needs, including those experiencing homelessness or housing instability.

Working Conditions:

Salary

$18 - $20 per hour
