Key Responsibilities
Provide comprehensive case management services including intake, assessments, treatment planning, progress monitoring, and discharge planning ASAM one-on-one counseling sessions, facilitate and/or co-facilitate groups
Maintain accurate and timely documentation within electronic health records (e.g., KIPU or equivalent system).
Supervise and support Behavioral Health Technicians, ensuring compliance with facility policies and client care protocols.
Apply knowledge of various treatment modalities (evidence-based, holistic, and alternative approaches) to meet the unique needs of each client.
Collaborate closely with the clinical, medical, and operations team to ensure a continuum of care across all levels of treatment (Detox, Residential, PHP, IOP.
Serve as a client advocate, addressing barriers to recovery, linking clients with community resources, and ensuring continuity of care.
Participate in multidisciplinary team meetings, case reviews, and staff trainings.
Compassionate, client-centered approach with a passion for recovery support.
Ability to multitask in a fast-paced treatment environment.
Qualifications
Current Case Manager Certification (CCM) or equivalent (CADC, RADT, SUDCC, etc.).
Minimum of 1-2 years’ experience in substance abuse or behavioral health treatment.
Demonstrated knowledge of ASAM criteria, treatment modalities, treatment planning, and evidence-based practices.
Strong leadership skills with prior experience supervising or coordinating staff preferred. Excellent communication and documentation skills.
