Job Overview

We are seeking a compassionate and dedicated Certified Home Health Aide (HHA) to join our dynamic team, providing exceptional in-home care to individuals across diverse needs. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools such as Hoyer lifts and other assistive devices.
• Monitor and observe patients’ health status by taking vital signs, recording medical information accurately in Electronic Health Record (EHR) systems, and reporting changes promptly.
• Administer medications following prescribed protocols while maintaining compliance with HIPAA regulations to ensure confidentiality.
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques and patience.
• Support clients with medical procedures such as catheter care, wound management, and hospice-related needs under the supervision of healthcare professionals.
• Assist with household tasks including meal preparation, cleaning, laundry, and maintaining a safe environment conducive to health and well-being.
• Document all patient interactions meticulously in medical records or electronic documentation systems to ensure continuity of care.
• Collaborate effectively with nursing staff, therapists, and family members to develop and implement personalized care plans tailored to each individual’s needs.

Requirements

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA), with current CPR certification.
• Proven experience working in home health settings, assisted living facilities, nursing homes or long-term care environments.
• Strong knowledge of medical terminology, medication administration protocols, and infection control practices including HIPAA compliance.
• Demonstrated ability to work with individuals with disabilities or developmental disabilities; experience with Alzheimer’s care, dementia care, hospice care or memory care is highly valued.
• Proficiency in using EMR/EHR systems for documentation and patient record management.
• Ability to perform physically demanding tasks such as lifting patients with Hoyer lifts or assisting with mobility safely.
