Job Duties:
- Conduct diagnostic evaluations and assessments to determine the healthcare needs of patients
- Develop and implement individualized care plans for patients based on their medical conditions and treatment goals
- Coordinate and collaborate with healthcare providers to ensure the delivery of comprehensive and coordinated patient care
- Monitor and evaluate patient progress, adjusting care plans as necessary
- Provide patient education on disease management, treatment options, and preventive care measures
- Utilize medical terminology and knowledge of medical procedures to accurately document patient information in medical records
- Collaborate with other members of the healthcare team to facilitate effective communication and continuity of care
- Rotating Holiday schedule
Requirements:
- Valid registered nurse (RN) license in the state of Wisconsin
- Strong knowledge of medical terminology, diagnostic procedures and treatment modalities
- Excellent interpersonal and communication skills to effectively interact with patients, family and healthcare providers
- Ability to prioritize tasks, manage time effectively, and work independently as well as part of a team
- Proficient in using electronic medical record systems for documentation purposes
- Valid Driver’s license
- Current automobile insurance
- Experience in hospice and palliative care, preferred
